I've almost constantly had a sub with PlanetSide since December of 2004...I haven't been playing it much recently, but it's still sitting there on my computer...
EQ 1998 - 2003. I occasionally login from time to time when I get one of those "come back for 10 days" emails from SOE or when I was playing EQ2 and splurged for a couple months and got the station pass. Other then that I would have to say Lineage 2 was the next longest span but its been broken up into chunks of playing for 6 months here, a couple months there, and so on.
While EQ was my first mmorpg, sadly i came in at the PoP era so that was about 2003, in dec 2004 i went to WoW, spent a total of 2+ years there on and off, i dont regret it one bit, while im bored with WoW now and have cancled my account WoW was the best thing to happent to the mmo genre. People think WoW is what started the Mmorpg easy mode decline their wrong. EQ started the MMO genre decline with planes of power, planes of knowledge etc.
Played Ultima Online the longest and SWG would be second.
Started UO in 1997 and finally left in 2002. Almost all my friends had long since quit.. but I was on the Interest team and found that more interesting than playing. (The Interest team did the in game Seer/Elder events..).
Think it was around summer of 2001 (?) that the lawsuit killed all the volunteer programs. So I stuck it out a bit but was very early in 2002 I left.
Well I've been playing Maple Story since it first came out to open beta, with Tespia as the only server. So about 4-5 years now.
I've played many games, and the ones that I play change weekly. Currently I am very interested in Gunbound, Poker, and Kwon Ho once more. I am thinking of returning to Maple Story and Ghost Online.
Consistently played, not just sub'd? SWG: pre-mounts -> one week after NGE. 5 accounts. Ah, the 5K run from the main city on Talus to our Guild town in the middle of nowhere, dodging Binyares, Foamers and the ocassinoal Devil-Flappy-Things all the way. Loved it. Miss it.

EQ1. started 1999 after friends finally talked me into it. Played for a short time til my old compaq cpu fried on me. came back mid 2001 and played til DodH came out. played other mmo's that came out in between but always went back to EQ1 even got my ex to playing it. then she ruined the game for me and i quit. realized i wasted my time and money on that game and will never go back to it. but i'm still lookin for that awesome mmo that will keep me hooked but on a part time schedule.

I usually run out of new things to do after about 6 weeks.
I tend to clock about 500 hours and move on.
I think 1 year is my limit. I played for about 1 year with WoW, UO, and EQ. No MMORPG can hold my attention for longer than that.
Doktar - 70 Troll Priest - Perenolde
Warp
Shadowbane -- 4 years
There are so many ways to make a character, I'm still making new ones
WoW for 2 years
AC1 for 11 months
DAoC for 6 months
SWG for 3 months
EQ2 for 15 minutes

Anyway that was the longest one.
