I was thinking about this in another thread and this is what I came up with for my ideal MMO game.
Updated and stylized graphics. Something similar to what WAR looks to have. I don't need nor want Vanguard graphics, but I also don't want WoW either. Something in the middle.
Realm Vs Realm system. I want the ability to PVP if I want, and I want it to mean something. Not the stupid ladder system that WoW has, but I also don't want rampant ganking and FFA like AC DT server was or pre Trammel UO.
The RvR system should have some type of rewards to it. Something that benefits the whole zone (like capturing Relics and Keeps does on DAoC) and something that benefits the individual. That individual benefit can be Looting your enemy (but I would like the AC1 system better then anything else. Give us the ability to protect our better items by lowering their value and carrying treasure that will drop first) or some type of character advancement based on RvR.
Random Object Generator loot. Like Asheron's Call 1. I want to go hunt and not know what loot I am going to find. I don't want some raids with the best loot. Let raids have cosmetic loot. Cool looking but not the most powerful.
I want a barter system with many different items used as currency and the main way to trade items is by bartering between players. I realize that people might find this annoying, but It really helps to keep inflation down.
I want a user interface similar to World of Warcrafts with smooth animations as well. That is one of the things I hate about Dark Age of Camelot. The graphics are nice, but the animations are crappy.
Lastly I want raid content, but I want it to have no cap on numbers that can participate. Similar to Public raids on Dark Age of Camelot and Public quests on Asheron's Call. I hate this 40/25 man raids in World of Warcraft.
So what does everyone else want?

Combine S.T.A.L.K.E.R , Hellgate London and Resident Evil.
City under quarantine. Radiation,demons and zombies everywhere.
Survived people has to fight cause the cant get out of the city.
There would be zones like in stalker and artifact weapons like in Hellgate.
Everything would be dark and in ruins. Constant night.
Single FFA PvP server with conquerable lands (improve game mechanics so that older characters are not guaranteed a win, and there will be less griefing)
Collision detection
Friendly fire on AOE damage
Player cities (but with restrictions on where they can be built)
No easy way of travel - ease of travel ruins many a good game. MMORPG are not CS.
Everyone in the game needs to eat and drink to stay alive and keep their stats up - this includes the npc's as well as the players.
Farming
Deep crafting system (think original SWG)
Skills not classes,
Fun not grind
No uber Loot
Raid are fine but should not be a focus
Good smart pve that does not focus on grinding...more story driven quest arcs that involve, travel, trade, killing and exploration.
Many many more NPC's - I kinda hate that there are more players around than ordinary citizens...makes it feel less like the players are the elite part of the game world.
Cities will have building that can be rented, turned into apartments, or stores...al sorts of things, like a real city

Player accountability - Characters and accounts are linked to a single person - if you are allowed to have extra chars or extra accounts then any player can find out all of your alts. I think this would curb some of the crap players do to each other.
Lots of other things too...but that is off the top of my head
territory control, 100% crafted gear, gear stats procedurally made(choose 1 step or 500 if you follow those steps again you'll get the same stuff), town/base building, territory control, skill based advancement, territory control, decision based combat(twitch sucks), low/unique fantasy back story, and incase I didn't mention it territory control.
I find it amazing that by 2020 first world countries will be competing to get immigrants.
I'll just say I agree with Heptah's list, though I would like to add that I think player run cities (outside of NPC cities) should require protection (can be destroyed / sieged). Only those dwellings in NPC towns that can be rented could avoid destruction but the space should be at a premium (have a high bi-weekly cost..if not payed you loose it. No grace period and if your account because inactive for any reason you loose it, regardless if your character has paid in advance or has cash in the bank).

I want the ability to change the game world. Not necessarily by myself, but with the coordinated effort of a group would be fine. For example the ability to change the creatures that inhabit an entire zone, or the ability to construct a bridge that creates a short cut, or opens a path to an area that was not accessible before, or the ability to capture a fortress, or build one, the ability to create unique items, or open up access to a dungeon, or close access to a dungeon, and things like that.

No one knows what Darkfall is gonna be, until it actually releases. Many people thought they know what Vanguard was gonna be, until it released.

The gameworld changing is really cool and I agree that I don't have to be the one that does it to make it cool.
For instance one of the coolest events in an MMO for me was when Arwic was destroyed in AC1. I remember logging in and watching people's mule characters fall to their deaths because they had logged out on the second story of a building that was no longer there. (and mule character's were generally overburdened so that caused massive fall damage).

Quest Rewards. Let's say you and your buddy are on the same quest. HE';s a clothie you're a plate wearing tank. Now in WoW the reward for this long long quest line you did turns out to be... a leather belt! Useless to you both!,, it must have proper rewards for the class
