EQ2 has an exchange server where you can buy/sell items for real-world cash. You can only do this on that particular server though. Some games offer special goodies that can only be bought with cash, no way to earn them ingame.
UNfortunatly there is (and seemingly always will be ) a MMORPG black market. This market isnt as 'black' as ones in real world, in fact some players think its fashionable in a "HaH! look at me, Im bad and dangerous...I just bought 300 gold on ebay" kind of way.
However, aside from Eq, I know that Second Life also allows significan transfer of RL $$$ to in game currency and vis versa

You can actually earn money in-game and transfer it to real money, minus a fee. Of course, the way the exchange rate works you would have to play for hours just to make $1 US, but if you want a game that is free to play and lets you spend real money to gear up your character, then that's what Entropia was made for. But then, it's not that good of a game lol.
Many of the games in the games list of mmorpg.com have "micro" in the payment column. Most of those are free to play, but require you to buy items with real money to get the good gear. That seems to be a common format with the asian games.
Yeah, EQ2 has the exchange server as others have mentioned. Just be aware that they are for American citizens only (so far). The legal stuff to allow such servers internationally are quite complicated I heard.

Everquest 2 has two (I think two) 'Exchange' servers where you can post Items and Gold for sale for RL$
The whole transaction is handled via SoE and is secure. They do, of course, take a cut of the RL$ sale.
