I am developing the perfect MMO for many of the hardcore MMO players here. I call it EverAbuse Online. It will be the most hardcore experience ever.
First, it will have Permadeath, to the EXTREME. Not only does your character die forever, but you credit card gets charged $20 every time one of your character dies, just to teach you not to be so weak.
The game will have hardcore PvP. You can attack anyone, anytime. No carebear cityguards, no sentry guns, just full-on PvP action. You can even kill people who are still in the character creation screen if they don't make their character fast enough.
Many games ignore hunger and thirst, not EverAbuse Online. You will need to eat and drink often, or you will die.
As this game is not for pansies, your character is dropped naked and starving onto a glacier (assuming you survive character creation). The only thing to eat is the flesh of other characters made at the same time. You will need to kill them and eat them if you want your character to last more than a few minutes. Is nobody else making up characters while you starve? Have a second account and make up a sacrificial character to make sure that your new character has something to eat.
One important survival tip, to avoid the polar bears, run faster than the other characters.
Usually, a bio break is only for the person at the keyboard, as most games ignore your character's need to go to the bathroom. For full immersion, your character will often need to go to the bathroom for long periods of time in EverAbuse Online. The types of food you eat determines how long and how often this happens.
EverAbuse Online will have a player-based economy, as the main resource will be the body parts of other players. Bone weapons, flesh for eating, and leather armor will be highly sought-after items.
Travel times between cities will be realistic- several days of in-game travel will be required to go from one city to another, assuming any players last long enough to try and build cities. In real life it takes a long time to walk on a glacier more than a few miles, so why should this game be any faster?
Any complaints about game balance and whatnot will result in $100 dollars being taken from your credit card, and your personal information will be sold to Chinese gangsters.
Does anyone have any other hardcore ideas for the game?

Also, I might up the death fee to 25 or 30 just to make sure people understand how truly pathetic they are every time some lifeless bastard 40 lvls higher than them jumps them in the character creation screen.

It was amusing though.
Crafting will be real-time also. And all items are player-made only.
A weapon will take weeks to complete. Armor, many weeks. And while your character is crafting items, he/she can still be killed via pvp. If you die while crafting an item, you have to start over.
Oh yah, all crafting materials must be bought from the MMO development company for real $$.

What genre is this going to be in.... I hope its a post-apocalyptic sci-fi fantasy with hints of FPS game play!!!!
Will there be mounts/vehicles?!?! Because if there is, they should be able to be shot out and perma die. Also they should have to eat/fuel up at ridicules intervals depending on how fast you have traveled...Plus you could determine travel distances by what kind of food/fuel you put in...Plus dont forget about tire pressure!!!

Or even better you have to be birthed into the world and survive coming out the pipe, once out you have to crawl around the world avoiding EVERYTHING and survive eating pocket lint and bits of string.

What about up keep of gear....That should probly cost you 5 dollars per item per week....that may not be hard core enough....but hell we arent all rich.

I lol'ed myself reading this, great write-up.
I'd give each starting character a massive bank loan which is immediately spent on "character creation costs". This way you can have compound interests piling up! The goal of the game should be to pay off your debt... Ofc, the said debt is in real money and no information is given about it until it grows massive at which point the debt collectors show at your door.
What we need is a term in the TOS for the game that stipulates that for the first ten deaths you get charged an escalating amount starting at $20 and ending at $1000, but that if you happen to die more than ten times, you give consent for the developers to send an assassin to kill you in real life. That way we can get rid of the scrubs, the carebears and the "people who suck" right at the source and get them to GTFO of our game, damnit!
Also, the game will come with electrodes which must be attached to your body to play. When you get hit by that fireball, you're gonna feel it, scrub! L2 LOS, loser! Haha!
Anyways, I like the ideas so far, but maybe you should add sleeping as a necessity of your character. 8 hours per day minimum! REAL TIME!

Yea! And it should be done as a minigame! So you can't cheat the game by sleeping at the same time as your character!

I have played every MMO that has ever come out.
Breaking Limbs physics.
A player can break the leg of another player. His character will be cripple for a few days until it's fully healed. Of course, during that time, he won't be able to wear a set piece armor on his cripple limb and his HP will be reduced to give sense to reality.
Broken leg = No running, Slower walking, no dodging, 2H weapon will hit half strenght, less balance
Broken arm = Less accurate, less damage when hitting with any melee weapons, cannot wear full armor, a knockdown will result in a reset to the healing process.
Broken neck = R.i.p
Broken rib = Each melee attack received will do 5 times the damage.
Broken back = Stuck on the floor and unable to move until fully healed.
Item decay and money sinks need to be added. You have to support the crafters or this game will die. I'm not sure if the permadeath is hard enough, maybe block the IP once someone dies.
Awesome! But it should be combined with the electric shocker device from a few posts back. If you break your leg you get an electric shock there randomly every 10-20 sec.
There really needs to be a method for sending, oh, I don't know, electric shocks down the physical internet connection from the virtual world to the player, so that the player feels actual pain when they're hit by a weapon in the game.
Of course, I'm not sure how you'll get this to work over bluetooth or other wireless connections, but there are plenty of nerds who love this sort of thing (you know, really hardcore MMORPGs) who will gladly devote time to perfecting the technology so the MMORPG experience is truly epic.
There is a serious problem with the concept, though, as player looting will be restricted to cannibalization, it seems. This will probably hack off the more serious sociopath PK types...they seem to think that unless they can loot some boss gear off another player, they're wasting their time. The thrill of the kill is not enough for them.

Yeah, we would have to add something like sharing the defeated players CC info with the PKer so that he can run up charges on the CC -- full looting in RL.
Make it skill based, but with a twist. 1010 different skills starting at 0% at character creation that can be raised to 100% each. Raising a skill by 1 % takes 1 USD being charged from your creditcard or takes two hours of grinding on average. Unfortunatly there is skill decay in the game with a loss of 1% an hour logged in.
Oh yes, there's actually only 10 real skills that have anything to do with gamemechanics, but they can only be learned once you have mastered the 100 prerequisite skills for each of them.
