I just picked this game up out of boredom since there really no MMOs out right now that can seem to hold my attention long, and it's actually pretty good. I wish more MMOs had a system where player decisions had consequences. I understand the consequences on the rest of the world would be limited, but the consequences for that particular character should be real and felt. For example, if you respond to an NPC in a certain way, they may or may not be willing to deal with you in the future. Right now, most quest givers give you two options: 1. Take the quest or 2. Don't take the quest. There's not much discussion in between.
Anywho, I don't understand why stuff like this still hasn't made it into an MMO after all these years.

Check out Age of Conan. The main story line in the game, called "Destiny Quests", put you on a path where you can make decisions...and they ar saying the outcome will be different based on the way you go about it.
While all the quests in the game are not like this...it is something interesting.
As for the Witcher...this review sums it up nice.

Yes, this would be nice.
On our NWN2-server it´s possible to annoy the NPC. Maybe they won´t sell you something for a while or such things .
But what really makes the difference is role-play. If all the other players role-play, it´s only natural your doings influence their actions and reactions towards you. It´s so much fun! No NPC ever will be able to make up for a role-playing fellow player.
I hope there will be a game once with bigger (legal) private servers. Like NWN2, but with maybe 500 players. This would be soooo cool!
