I'm curious about voice chat in D&D Online. They have it for lotro, so it would seem to make sense for D&D Online. Does DDO have built in voice chat? If not, do guilds frequently make use of ventrillo?

Yep, voice chat is built in.
Yes DDO has the exact same integrated Gamespy VOIP that LOTRO does. The difference in DDO is that far, far more people actually use it compared to LOTRO where, in my grouping experience, most people had no idea it existed.
Everyone uses voice. I don't think we have a single guild member who does not use it although there's a few I wish wouldn't. Eh, just kidding. I'm in a great guild with some good people of my own age - around 20+ active members with ages 25-45.
Ventrilo and Teamspeak have been mentioned although the accesibilty of the voice coms in DDO are too seamless and many players don't want to bother with other programs while playing.
I've never been able to get the ingame voice chat to work, yet it works fine in other games, doesn't like my onboard generic sound drivers methinks...
I prefer to use TS or Vent at any rate...

Absolutely! I have no issues using TS or Ventrilo but I'm more a techie. I lost count how many times back in WoW I had to take a fair amount of time explaining to guild mates how to install Ventrilo, where to put the IP:port information, what all the options mean, how to configure their microphone... they may have better quality than Gamespy chat but their user-friendliness for non-techies leaves much to be desired. Integrated VOIP is ready at the click of a single option. It *just works* end of story. Plus in the 3rd-party solutions, you don't know who's actually talking (assuming you don't know everyone's voice already) unless you're playing windowed (ewww) or have a dual-monitor setup and stare at the VOIP window every time someone speaks.
I'll take integrated VOIP any day over a 3rd-party solution, and I hope Vivox is as good as they claim it is and more companies start using Vivox rather than older tech like Gamespy or making their own VOIP like WoW and TR did.

I use the Logitech G15 keyboard and an applet for the LCD that shows who's actively speaking on Teamspeak. The keyboard also has 2 USB ports conviently placed for the reciever on my wireless mouse and the X52 HOTAS. the wrist-rest is very comfortable for typing and gaming and can easily be removed.
