Last night was my first experience with using spens. I picked up 3 spen drops quite quickly (just a fluke thing). Instead of selling them, I decided to upgrade my mace which hit for 22-26. I gave the Blacksmith the weapon, spens and I think 770 gold and I got back my weapon with 25-29 hit stats.

The the effect was one hit point for each spen, this may change when more spens are required.

I noticed the difference. Based on the cost of the spens, I probably would have been better off selling them, but since I got all three in 10 minutes, I figured it was worth a try.

I probably would not use any more before getting a weapon that requires str of 40+. I am glad I did it just for the experience though. My character has no future anyway and I just learning for when I can get on the NA servers.

Upgrading is really worth it, especially if the thing you're upgrading is already good for its level.  All green stats on the item and upgrading them make the item worth far more then unupgraded.  Selling spens and the like to the shop is a way to get quick cash.  Yeah it's nice to do, and with the almost unlimited supply of spens from the arena it's easy to get a lot of money fast.  What I really hate is the fact that people try to rip you off selling spens for 50-10,000,000 each.... it's just stupid.

Upgrades to armor and weapons +4 and above make your items WAY more valuable.  Also the stat increased and slot increases multiply with every second upgrade you do.  +4 gets you 4 slots to add gems to for example.

Hints for successful upgrades, use Mitchels for the second and third upgrade, and yeterni for the 4th.  You might also want to try your upgrades on the third island, somehow this might be better?  I still don't know about that yet, but my armor attempts have had more luck there.

From my experience its okay to upgrade to +2 for low lvl items. Even that much can make your green stat low lvl gear last a long time. The merchants have breaks in the items they sell. e.g. the best sword basic merchant has is a two handed sword that needs 39 str. Now you go the advance merchant and lowest str. two handed sword he has needs 56 str. Theres a huge gap there and the only way to fill it is either have a hard time lvl or to upgrade your weapon. At mid lvl which I consider to be 50 you should consider getting +4 on all your items. And end game aim fo +6 on everything. Also every two upgrades adds 1 slot to the item. So a 2 handed sword starts with 2 slots. After two upgrades it now has 3 slots. So at +6 it would have 5 slots. And the damage goes up every upgrade by 1-3 pts I think. What I do is add a black moon to the first slot to give absorption damage than add rubies for the rest which adds to min/max damage. This is for a warrior. Other classes need other stats more than damage. Like the sin would probably want critical hit rate which comes from diamonds.
