OMG come on guys, we can't forget the best launch title of all times. The one that had nearly no flaws at all, thats right I am talking about....
Anarchy Online
ROTFL, sorry that was just being sarcastic, this was actually the worst launch of all time. Hell the beta was one of the worst of all times as well. Anyways, best launch that I can think of would be a tie between,
LOTRO, winner and still champion! Smoothest launch I've ever seen, and then they just kept adding content every 8 weeks or so, increasing world size, adding quests, hobbies... no other game even comes close.
Lotro had a good start yep, wow had for me the best open beta so far wheras the start wasnt that good but still enjoyable, vanguard was craptastic for me. No idea how did the other games at the start.
How anyone could say WoW is beyond me. Have people forgotten the constant servers being down the insane lag and the constant roll backs for the first 2 weeks or is that just me and icecrown? Even its expansion caused major issues with server stability and site stability and prevented people from putting in their upgrade codes for days if not a week+.
AoC? Well you can just go to the forums and see it didn't have a happy launch heh.
All the rest shrug, LoTR:O got my vote due to their smooth transaction from open beta to gold to release. Only minor lag issues in the major cities, and people were able to get on and register their stuff. The vendors, npcs, and banks all worked properly and most importantly it didn't crash. Though they did have an emergency maintenence for 2 hours on release night but then nothing after. It was probly the best release I have ever experienced, and hope other MMo companies can duplicate.
I say the best launch ever was Ultima Online. It was obscure, without all the expectations (and subsequent whining) that comes with modern MMORPGs. It was so new, that people were just blown away by the concept, and the bugs were simple expected for something so different. This is perhaps a load of semantics, but I've seen quite a few mmo launches. UO was the only on I can remember were the people playing it weren't bitching, but going "Holy crap, this is the coolest thing...>EVER!"

You really should add the titles before 2004. Coh did have a smooth launch, L2 was pretty good, DaoC didn't. Of the ones on your list Lotro's was damn smooth, but my best experience was with Asheron's Call.. It also had a great end of beta .
WoW was pretty instabile the first year. Daily crashes, ships disappearing dying of fatigue in the ocean. Getting killed in Barrens and turn up as a ghost in Silverpine Forest. Damn pet pathing problems. ( Played Warlock ). Falling through the world.. dunno how many times. -.-
I didn´t envy those that tried Molten Core before that place was debugged.
When there was Alliance attacking = laaaaaaaaaaaaaaaaaaaaaaag.
Forgot the underwater bug with Ati card. Rare but when that happpend you crashed out and couldn´t log back in with that character. You had to log in with an alt and ask a GM for help. Vile reef in Stranglethorn I hate you... -.-
No crashes. No emergency downtimes. And amazingly- they did the free content updates every 2 months from launch till Christmas. And Turbine's "Patches" are vastly different than other companies. Their free content includes new lands...new quests...new armors...houses for the players, etc.
That company busts their butt. I sure would like to know how much coffee is drank in their building LOL!

Didn't vote as I wasn't at the launch of any of those games. So far as CoH goes, it was relatively smooth, no crashes, the only heartache was the serious amount of rubber-banding that would occur.
Earth and Beyond, no server crashes, no lag and no client crashes. Only issues they had was a trading exploit that allowed people to mass mad amounts of money and trade xp on a 1 jump trade run.
Well at least on my server, and I played on the most populated server at launch.
This newest generation of gamers sadden me greatly.
WoW. Based on the quantity, quality, and number of initial players. LoTRO second. DAoC third. CoH fourth. All others running far behind those four.
WoW was, well to put it bluntly, fucked up at launch, basing it's launch success just on the number of people playing is stupid. Multiple server crashes, login issues that lasted months, falling through the landscape, I could go on for hours with WoW's issues at launch. However I'd be willing to bet you're one of those fanboi types that can't find fault in anything related to WoW.
As for the others they did Ok. LoTRO was most definately the best out of the list you have there, however again I state the current generation of gamers saddens me greatly because your knowledge of games flatly sucks. Most people like yourself think those are the only MMO games to have ever come out, and the few of you that do acknowledge older games, like UO never even touched it but think because it's old and has sorry graphics it must suck.
However UO's launch did suck. It was however the first true MMORPG, and everything it did was a testbed for every game to follow. So every step they took was new and when you do something totally new you're bound to screw up.

for me LOTRO was the best launch ever.
missing City of Heroes and Dark Age of Camelot
- they both predate 2004

Asherons Call and Cabal Online
I just never had any issues right from day one.
LOTRO, hands down.
WOW? LOL. Their servers went down after launch more than a $10 hooker.
LOTRO, albeit a very dull experience it just worked.
LOTRO, winner and still champion! Smoothest launch I've ever seen, and then they just kept adding content every 8 weeks or so, increasing world size, adding quests, hobbies... no other game even comes close.
When scary things get scared, that's bad...
Who did vote Vanguard ? LOL
Lotro had a good start yep, wow had for me the best open beta so far wheras the start wasnt that good but still enjoyable, vanguard was craptastic for me. No idea how did the other games at the start.
Cabal Online had a great launch, unlike the others listed it is a F2P game but I almost couldn't tell that they had gone from Open Beta to Live.
Given the offerings I voted for LoTRO
?
*bump* for more votes
From those listed I'd have to go LOTRO.
Until you cancel your subscription, you are only helping to continue the cycle of mediocrity.
LOTRO without a doubt. No contest.

From what I heard LOTRO.

Want a taste of religion? Lick a witch.
You want a game with a good launch ? Don't look at a mmorpg. Look at Guild wars and look at online action games. CoD4 perfect launch date.
The least flawed launch I can think of for a game is a tie between LoTRO and POTC online.
WoW. Based on the quantity, quality, and number of initial players.
