Unfortunately it seems that gaming companies don't really care about enforcing the name policies. Most enforce the rules for maybe a few months and then gradually slack off.

This is why Blizzard will be the death of MMOs. Back on the naming policies, if someone reports a fellow player with a violation in their name it should be changed. It wouldn't be that hard for a CSR to change it, but for some reason they will change one here and one there just to keep people from a meltdown.

I know that in WoW many times bored kids will, when their PvP server is down, go onto RP servers and create toons with obviously rule-breaking names, and engage in asshat behavior on those servers. The n00b zones are flooded with twits who are racist, sexist, and trying to be as offensive and obnoxious as possible.
The only suitable punishment for such types is to deprive them of ALL their toys. As in your account is cancelled. We don't want your money.
But, the MMO bidness being as it is nowadays, even WoW will not take that step, because they love the green stuff more than anything else.

Players can always find a way around naming filters. Personally I'm more tired of seeing X in front of and behind a character's name than I am of seeing "Mr Hawtpants." I can suspend my disbelief and say that perhaps Mr Hawtpants is his nickname. However, what person has an nickname xXBrianXx?
Yeah, that sort of thing always bothers me. But, what can you do about it?
SWG's naming regulation filter sometimes took things in the other direction, and actively encouraged "workarounds" to the filter system. For example, try to use the string "han" in any mobile object in the game. I could not name a savage humbaba pet "Concorde" becaue "corde" is in the filter because it's the name of one of Amidala's decoys, the one who gets blasted to nerf dust in the opening sequence of Episode II. So I had to name the pet "Concord", dropping the e at the end.
Yet, I have three toons on three servers named "Sio Babble", which if you substitute an I for the a in Babble is the name of the Governor of Naboo seen in all three of the prequels.
I guess since John Ford Davies is not some hot chick, is not a sexy character, the SWG naming filter braintrust figured no one would name a toon after him.
Well, they thought wrong, and the bio of Sio Babble on Ahazi explains why he's named that way.

Well i completely dissagre with all of you guys, i'd rather see a name such as "Justin Case" instead of an unpronouzable, unmemorizable name as "Elthariewyn Glyfgreo" for example. At least with the first you may smile slightly. But thats just my own opinion and I understand that it bothers some people in game.

Unfortunately if the good names run out you may get stuck with crappy or silly names.
Hopefully they will allow a 1st and last name to allieviate this problem

It would certainly add to the immersion on rp servers, if frp naming criteria was used.
