Here we go again.. LF difficult game.. Non instanced PVP
World non-instanced PVP. meaning NOT L2 or guild wars plz. If I follow someone out of the city they DONT DISSAPPEAR
Hard to level, Hard to progress.. dont care if its grinding. No getting to max level in a week BS.. No level = even better. No limit = best but I doubt thats gonna happen.
Good graphics meaning architecture, scenery, dungeons with corpses hanging on the wall. so NOT EVE.
Already released.
Thats it! surely there is.. something out there that fits the description?

indeed there is!
its still in developement but should be entering beta by October. Beta sign ups are open right now.
Darkfall boasts among other features (there are alot)
- No levels - skill based system with 500 skills and 500 spells.
- The largest game world currently created (8-12 hours to cross)
- FFA pvp
- Full loot
- No instances, the world is unzoned and seamless. The only zoned area's are dungeons BUT, they arent instanced, other people can come in.
- all items are craftable
- real time combat, Aimable arrows/spells/strikes that can be dodged/blocked.

After reading your post I surely remember a few games which match the similar criteria.
Perfect World is a hard to level game and the constant cost of buying hierograms at higher levels makes it even economically tough. But there are limited dungeons and the dungeons are not very public dungeons also. Graphics is good and there is immense depth in crafting and collecting raw materials. PK is an attractive part of the game and the players do not vanish until you or the person who attacks you kills you. In a nutshell if you are looking for solo grind and tough game-play then I think perfect world is a good idea. The whole idea of leveling in perfect world is somewhere related to PK and it is refreshing to go out and kill a few bots or may be players.
Apart from this I still have not seen something with dungeons and tough leveling. Whenever there is a public dungeon leveling seems to be easy for that game and soon you may reach a good level and from there the population thins out and ultimately what we have is a high level toon with no content to seek.

Seriously, did you not read the post the OP set up? He wants a game that's been released already.
I'd recommend Everquest 2, as it does fit all your requests. Then again, I'm biased.

hmmm, try Ryzom:
- skill based. max 250, over 50 skill trees. after 4 years only 25 been mastered by few people
- huge and dangerous world (Silan and mainland completely different things). Don't know how huge is it cause I'm myself started few days ago) But 5-6 hours of walk thats for sure.
- ALL gear are player crafted.
- great crafting system (if not the best one...)
- ok graphics
- non-instanced PvP( faction,guild(outposts),arena)

Yea darkfall.. I signed up for beta. i'm beginning to think I should just wait for it, only I dont know how long that will be.

I have tried ryzom and it does seem ok, I will continue to try it although it hasnt grown on me just yet.

Man I hope darkfall is decent. WoW is a joke.. pvp in a bottle, all predictable. I care not to discuss that, all I know is that I dont enjoy it.

I was going to say Ryzom, but you know about that already. You said you only wanted games that are already released so I wasn't going to mention Magic of the Gods, but now you say you're considering DF even though it's not released yet. Well in that case, Magic of the Gods is sort of released and sort of in beta or even alpha depending on your definition. The way they're doing it is definitely wierd. But it's a good game.
Graphics aren't the best, but you want a game that's hard? Some things in MotG are easy, but the really worthwhile ones are extremely hard. Since the game is so new, most of the really impressive things you can do, nobody in the game has actually figured out how to do them yet. For example, wizards can invent magic spells, but I think only two players have figured out how to do that. Wizards can also create new kinds of magic items, new kinds of creatures and even new playable races, but nobody has managed to do any of that yet.
I'm trying to create a new kingdom. Nobody has ever done that either, but the game devs have repeatedly said it's possible. Not only am I going to make a new kingdom, I'm going to do it with a race that doesn't have any kingdoms so far. That race is the humans. Oddly, humans are rare in MotG and apparently are on the brink of extinction. We'll see about that. If you do try MotG, look for me online. My character is Radnar.

Seriously can you stop mentioning your stupid realm crafter game, motg's. You dont even have a website, or full server yet. STOP TALKING ABOUT THE GAME YOU ARE HOSTING IN YOUR PARENTS BASEMENT.
You mention a homebrew game in every single thread here, your not fooling anyone.
