Well basically it all comes down to how fast different retaillers get teh game from the distribution center. Places close tot eh distribution centers are naturally going to have them sooner. Some retailers go out of their way to get the game as soon as possible, I know some places meet the shipment at the airport and take control of it there rather than waiting for it to go through the whole Fedex or UPS or whatever process.
Also I believe opening the servers before most people have the game and not announcing a set server lauch time was a good decision. It allows people to trickle in as they get the game rather than how some games have done it in the past, announcing a set time for the servers to go live, then watching in horror as the servers get absolutely hammered as everyone in the world tries to log in at the exact same time.

No, the disoveries are YOU discovering that area for the first time, the first time you go to certain areas you get credit for discovering it, basically it shows who has explored more of the world. Yeah, some people are getting a slight headstart, but in the long run it won't affect things tooo much other than making the launch go a lot more smoother in my opinion.
