As far as PVP goes the best games are probably EVE, WAR, AoC. Those are just the current ones. My favorite PVP mmo was DAoC but with its very low population it wouldnt be worth it to start playing again
PvP games with PVE aspects are all the ones i just mentioned there but you could try WoW out if you havent already. Thats more of a pve game with pvp added. Class balancing needs working on in pvp in that game...a lot.
Just do some searching and look for MMO's that suit your interests. Then research them to see what people are saying about it. You might have a 10 day trial, which i know there are some for i think all of those games ive listed, so give it a shot.

as to pvp game, i come to guild wars and shadow of legend, both feature open pvp.matching, joining in the guilds, running to do challenging quest or guilds VS guilds give ppl lots of fun . and in the shadow of legend, running duel in arena not only gives bragging right, but also honor points,which can be exchanged for items in the item mall. hope this two will help.

The next one may settle for nothing less than D2 Nightmare mode. It's too varied to say any one MMO is the best for PvP.
Post what type you like and there'll probably be plenty of viable suggestions for you.

WoW is a distant second in this category, its certainly not as balanced, but it is a better option if you want a game where the PVE is solid as well.

WoW is a distant second in this category (not counting DAoC), especially Wintergrasp improved mass world pvp enjoyement. id honestly put lineage 2 up there, even though many people claim its skills were broken and hate that game...i thought the castle sieges and open world pvp was actually pretty solid.

Ragnarok Online has good PvP as well in the form of War of Emperium. Basically there are limited castles in the game world which guilds fight for. Obtaining a castle will give the guild rewards in the form of income via super rares and items only obtainable via castles each castle having unique items.
If you're looking for PK the only popular game on the market right now which has good PvE in it as well is Lineage II. This game has been infamous to western gamers for what has been percieved as insane exponential grind however the recent update has added a sort of vitality system which will speed up leveling time described to be between 3-5 times faster so you can reach the level where mose moderate (not hardcore) players are at within a month or two. Also this game releases 3-4 major updates for free which other games would call "expansion packs" and charge for.
Edit: Oh you want full loot ey... well theres one thing i have to say about that. Full loot games at present are one of two things. Very dated game (Does not mean its crap) or very unpolished.

Shadowbane
This is probably what you're looking for also an extremely dated game, recently became free to play. In this game not only is there full loot, you can actually take over the world. However when I last played most of the servers were controlled by this one same unstoppable immense clan.
Darkfall
I have not played this yet and it is hard to obtain a copy of this game so I cannot comment formally however from what I have read and watched the only way I can describe this game to a person who has not played a sandbox styled non-leveling MMO is that this is MMO Elderscrolls (Arena/Daggerfall/Morrowind/Oblivion).
For best small scale PvP it'd be silly to not mention GuildWars. We're talking about a very dynamic, rich, and balanced class system after all.

Guild Wars has an absolutely fantastic PVP system. PVE though is a joke... you can level cap in a day, and then its basically "make money for the rarest items" game.... after you unlock all your skills of course. That said, the PVP aspect and missions (IMO the only worthwhile PVE content) are very well done. Take my advice with a grain of salt, I haven't played GW in about 1 1/2 years now... perhaps the PVE has improved.
To be honest, what bugs me most about Guild Wars is that it's almost completely instanced... I absolutely hate not having an open world that I can explore. It's completely linear... I'd probably still be playing if it wasn't for that, apparently Guild Wars 2 WILL be more open and oh boy Ill be playing from day one
Addicted to pvp mmo, i choose shadow of legend to be my favorite.
No matter i'm in camp or facing the opposite ppls, we can start pvp whenever we like. Another amazing point is that, an item called "Marauding Flavorant", allows you to fight anybody from players to NPCs, and take their dropped items. Be warned though, when using the "Marauding Flavorant", any player or NPC will also be able to attack you.
