Is the game getting any better or is it the same ole same ole ? Last time I played the starting zones were pretty scarce and such a pain to even complete good pqs.

If you roll on a populated server right now you can find action. Scs pop pretty much non stop on my server gorfang during primetime, and even going late into the night they will pop occasionally. Pqs do get done as well as rvr.
+there are better rewards for these goals.
The game is on a rebound and is working much better. Patches are coming out faster and fixing more and more, and improving other things. Now is the time to come back. The servers listed when you login to the game are the most populated, Dark Crag is where I am and it has pqs and scs popping all the time.

They are down to 6 servers in North America, they are merging Oceanic with NA servers too.
IMO, it s much better. T4 is great now.
I rolled a chosen (melee dps) and still in T1. So far not too bad but it still amazes me how people still can't grasp the concept of capping points in scenarios. Anyway, it seems I'll be sticking with the game for awhile until Borderlands gets here and perhaps I'll still casually keep playing.
Chosen is a tank.
