Do this game follow Coh/CoV? In term of zone size? Or is there a bigger feel to it this time around? What am essentially asking about is the world open and free? Or is it closed off with a lot of instances? Or maybe something in between?

My opinion is that it is something inbetween. The Zones are larger than those in COH but they are instanced. In the tutorial and advanced tutorial (called a crisis zone) they are boxed in as part of the story line. Once you are past those it opens up quite a bit. I had only gotten characters into the low 20s in level during beta so I did not get to Lemuria or Monster Island but the three primarly locations City, Desert, and Canadian zone all had loads to explore and are very large.
With the cool travel powers coming into play earlier on (around 5th level) it does make getting from end to end in the zones easier. This might give you a sense they are smaller but they are not small at all.
Like COX, there are instanced "dungeons' as well within the different zones.
Its not instanced like CoH. When you finally get to lairs those are instanced. The world is all on one server. Pretty much everything happens in the world itself.
Well in Millennium city it is kinda similar to CoH. I've been running some quests involving killing stuff in the world but also quite a few instances esp from people that you rescue or that run upto you on the street. Most of the instances are smaller then CoH instances and don't actually have any bosses. At least this is my experience so far at lvl 18.

Its not instanced like CoH. (Every mission is instanced pretty much in CoH). In Champions most missions are out in the world.

The point I was trying to make is that in Millennium city there are quite a few instanced missions like in CoH but the instances are quite a bit smaller. I'd guess that maybe 50% of the things I've done in MC are in instances? So if you like doing CoH instances there are some in CO although so far I haven't seen any that are repeatable only ones that you get from random quests from street people or as part of normal quests.
But you are correct unlike CoH there are lots of missions esp in Canada and Desert zones that are non instances and are more like WoW or a normal MMO where they involve doing tasks in the world.
There was an instance in Desert zone that I did which I was able to go back into but when I went back into it there were no mobs to kill... Maybe if you wait awhile it might repopulate but I didn't come back and try again later. Unfortunately getting xp from killing mobs is pretty pointless as you would need to kill 1000s to level up from the tiny xp you get from them. If you want to grind in this game they only place to do it is doing the open quest areas.

There is not that many. There is civilain Missions and Bank Robberies that come out to about 1 out of 5 missions (some you can do over and over again which makes it look like there is a lot). Then there is the ending Lair on some of the higher ranked mission.
