All but the towns and forts and other such "meeting locations" are persistant, only people you'll ever see in the world are the people in your group or NPCs unless you're in a city, all the mission zones and explorable areas are instanced.
EDIT: Corrected since I was apparently smoking crack when I first answered this morning.

Ehm.. kinda semi wrong reply there Kunou. All the meeting places aka. towns, outposts and sucks are stationary places. Only the diffrent maps/ misssions that are instances. Im pretty sure that´s what you ment as well.. But just wanted to clarefy if anyone els got confused by your anwser as well. Maeby its just me thats tired.
Fixed, yeah, I just got my words all mixed up... It was early...

well, its not really that complicated. First of all, there is no PvE (player versus Environment) that you can do without your team. Everytime you leave a city or town, you automaticcally create your own map, only for you and your party. This is like D2. But it is not like D2 when if you go to a town, and then go back to the same outside you came out of, the creatures that you killed last time will respawn. Hope that clears it up

"If I came here to kill you....you'd be dead."
