Game looks interesting but judging by the videos i saw, it looks like you need to constantly click the mouse buttons to attack is that the case ? I had carpal tunel surgery a few years back and games requiring to constantly press mouse button are bad for me. So how is it ?

Well, seems this game is not for you then, or you could try a gamepad, they are more ergonomical when pushing buttons..
Personally i wonder if its super important to click as fast as possible or that there is more in the mouseclicking buiseness...

Some weapon skills like rifle don't really force you to click much at all, while others like staff would have you clicking a lot more. As you get combos, you will click a lot less.
Controllers would help with the clicking, but as said, its not about how fast you can click, all attacks have animations that you have to go through, so you can only attack as fast as your weapon animations allow.
Definitely not good for carpal tunel.

DCUO is to button mashing, as SSF4 is to button mashing.
This game is functions much better with a controller.
It is very clicky, no question. In a good way, in my opinion, but not if you have CUTS, definitely. A controller might help, and those who like controllers praise this playstyle. Otherwise, frankly, take a pass and play something with less action. No game is worth screwing up your hands.
