I'm not really into PvP so I wouldn't know for sure, but... aren't 1v1 fights to the death from a fair start incredibly uncommon? Either you're in some neutral battlefield with a bunch of your friends against a bunch of enemies, or you and some allies are in enemy territory taking on single targets until a team of defenders comes out to oppose you, or you're in an instanced battleground where it's team versus team. In the unlikely event that you do engage a single target, it's a given that you're going to do so in a way that puts them at a disadvantage, either by ambushing them when they are low on health or in the middle of a fight, or approaching them in stealth mode so you get the first attack. The concept of two equally-prepared players approaching each other with intent to fight it out on an even playing field seems like a very very small slice of the PvP pie.

What do you mean by RNG? Highly dependent upon gear? Critical hits? CC proc rates?
Does anyone else find it ironic that the OP wants PvP without RNG, but wants PvP based on player mistakes (aka luck)?
