Team Fortress 2 is hands down the best war themed hat simulator and it went free to play recently.
Are there hats?
Battlefield Heroes is free but you can buy hats with real cash that give you stats same with battlefield free2play. Battlefield heroes is more a cartoon game and battlefioeld free2play is more realistic
There are more hats than you can possibly imagine, and you can get them from random drops, crafting or you can purchase them.
Are they simulated?
Well of course they are.
How is it war themed?

Good question. Well to start there are two sides, The Reliable Excavation and Demolition (Red) and the Builders League United (Blu). For countless years Red and Blu have been at war over land left by the leaders of both, father. When Redmond (Leader of Red) and Blutarch (Leader of Blu) couldnt fight any more, nine people, the Scout, the Soldier, the Pyro, the Demoman, the Heavy Weapons Guy, the Engineer, the Medic, the Sniper, and the Spy are recruited by both sides to continue the war.
Thats the basic jist of it.
So is the focus on the hats or the war?
Well a bit a both actually, while you directly take part in the war a third party, called Mann co., creates and distributes hats, oh and weapons. So while you fight the war, the worlds leading distributor of hats, and weapons, is giving hats out to pretty much everyone, and also giving weapons.
Because when youve gotta kill a man, you should do it in style.
(I am not an official spokesman for Mann Co. or its hats... and weapons)

Hmm well I cant think of anything thats hat focused, but if I come across something I'll be sure to let you know.
How about Red Hat linux with Gears of War theme? Or Red Hat linux with Star Wars theme? What kind of War you want your Hat with?
You could always just buy a white baseball cap and write "war" on it.

I've got to admit, I'm slightly confundled on this one. As it stands, if the OP was looking for a war themed hat simulator, not expressively indicating a war simulator of themed hats (my pirate hat takes your fedora) then I'd stand to reason there are plenty of war themed simulators indicating explicit hat involvment.
However, my wonderment comes from the possibility he is looking more for a Simulation utilizing a hat war theme (something the Pentagon has been dreading for years). Whereas I have my own suspicions on the matter, it would be ill advised to discuss it or its implications, especially regarding (but not limited to) styles of hats, hat attack formations, or any occupation of the military persuasion by ANY major form of head covering.
Now, if this were... say.. heh.. like.. heh.. like a post looking for a War Themed Glove Simulator.... well... we'd be in a different boat.

So you want to emulate war themed hats? Why the obsession with hats? If you are looking for images google is your friend. Im sure you can find many images of war themed hats.
Why would you need to emulate war themed hats though?
If you gave us some idea of what you are trying to achieve we could probably help better

The current crop of war themed hat simulators are somewhat dumbed downed and for the masses.
What you can do instead is look to having your current choice of mmorpgs changed into war themed hat simulators. File a petition in whatever mmorpg you play and demand that the server be completely wiped, all characters reset, and the game design changed to be that of a war themed hat simulator. Keep filing petitions until they listen to you. Remind them that you pay a sub just like everyone else and that they should change their game to suit your desires.
Darkfall is already about to do 2 out of 3 of those things I listened above. It wouldn't take but 2 or 3 petitions by players to get them to change it to the game you want instead of the DF2.
If all else fails you can just sue the companies for not complying with your demands, then you can take the money you get from the lawsuit and make a pay to win browser game.
