From my understanding (things might have changed), the only common area in the game will be the city of Stormreach (is that right?), and all of the quests will be instanced, highly detailed dungeon environments. In regards to the outside world, however, the devs have stated that there will be some of these quests that take place outside as well as inside dungeons.
Also, I think I remember reading something about the possibility of being attacked 'en route' to a dungeon, a la a traditional D&D random encounter.
Someone else might be able to provide better/current info, though; I haven't been keeping up with the game as of late.

I think the devs also mentioned that there will be a very limited number of "open dungeons" besides Stormreach (the city), but there will not be an open world. They call every map a "dungeon", but that includes both caverns, underground, forests, jungles, etc.
So as far as we know, all there will be are instances and the main town or am I wrong, because I certainly hope I am. As nice as it would be to have all the players together, I don't think I could ever play a game where all there is is a town and then dungeons to go to from inside that, I need to be able to explore and run around.

I think they are atempting to make it easier to find groups. The intention is not to have solo content, I believe which is what exploring areas and just killing mobs causes. There will be heavy emphasis on group ojectives.
I like the idea, actually. At first you're like "WTF?". But come to think about it ... Traveling to other towns constantly sucks. The first time, you get to see a new world, it's kind of exciting. But traveling back and forth between towns is quite literally a time sink. There's nothing an outdoors environment provides that dungeons can't. I think they realized it's impossible to create the tension you get in a pnp game or even a text based mud by traveling. You know exactly what is where and when it's going to do it in an mmo. Dungeons, on the other hand, those can be changed constantly.
Being a lifetime D&D fan I unfortunately fall under the category of "Would by the game if they filled a box with crap and labeled it D&D: Online".
Now isn't this a similar concept that is used in Guild Wars where everyone is in the City/towns but only yourself or those in your group are in the instanced areas/dungeons?
I am aware Guild Wars is not D&D and one can only hope it is nothing like it. Just stating a question about the similarity between the instancing and mass meeting areas.
