This is what I'm working with, from this site's game list, filtered to show only fantasy games that are currently released, and subscription based. The red ones I've already ruled out, the green ones I'm already thinking about, and the white ones I don't know enough about, so If you know about them, and think they can meet the criteria below the list, feed me some info please.

-Has at least 5000 active players per server.

Ultima Online has been one of my favorite MMO's since 98. Unfortunatly I've played it so much I've worn it thin and can't get back into it. Though it is a good game if you can deal with the dated graphics and not a typical level type mmo. I'm a little tipsy from drinking but perhaps you should give that a try. Has a free trial. There are a few others that are on your list that are interesting.
I say, take a look at the all, youtube them and watch gameplay, decide what you want to play.
Some thoughts:
FFXIV is still in the process of major changes to fix what was a completely broken game on launch. I don't know how many people are playing it now but you'll probably want to wait and see how things go before you jump into it.
Aion is not PVP only, not sure why you thought it was. On the other hand Warhammer IS mostly PVP so you'll want to rule that one out.
I find it hard to believe that you never saw another player in Vanguard. Seemed like there were plenty of people around last time I was on... do you just normally play off-peak hours or was it one of the less populated servers?
I would say Everquest, FFXI (it's in much better shape than XIV right now), Aion, and Vanguard fit your criteria well enough. I'm curious as to why you're ruling out F2P games though, especially ones like LOTRO which are basically P2P games which have traded their subscription for more players and the ability to pick and choose what you want to buy. It's probably cheaper than keeping up with the never-ending expansions of Everquest. You could even just subscribe to get everything if you prefer that.
Edit: Forgot to say that if you're really against F2P that'll knock Lineage 2 off your list as well. It's subscription based now but they announced that they will be making the move to F2P soon.
You can scratch Minions of Mirth off your list. It is more dated than EQ except it has far lower quality content and much less content. It has virtually no population. As far as I can tell, it isn't an mmo in the normal sense of the word. It's more like a half hearted attempt at an mmo that never fully materialized.

Spells in the D&D universe are gifted per day/rest cycle to keep spell casters from being too powerful.
They are meant to be played a bit more strategical than a spam-mage you might know of many other games.
(edit) Since you're into healers, DDO requires LoS to your wounded/disabled target. It can be a bit tricky in fast fights when people move around quickly but with guildies/constant group you might figure things out. It's fun! (edit end)
In DDO they added a blue bar instead of memorized spells to keep it more flexible.
That said, many dungeons have rest shrines to replenish your mana.
DDO is one of the better games btw.
Same for LotRO, farming TPs is an alternative for paying $$$
