So i understand you never know what loot you will get from drops because the loot table can drop various things per monster and ites like barrels but I have a couple other questions.
When you enter an open world area with monsters if you clear the area and then go back to town, the next time you go to the area is it filled again or still cleared?
I heard in Diablo II the dungeon layout can be different each time you go in. For Diablo III is it different each time you go in the dungeon or only the next time you play through the game (i.e. layout is set after you go in the first time)?

I got burned out on the beta a few patches ago, mainly because you were limited to level 13 at the time (I don't know if this is still the case).
At that time, the general layout of the zone map remained the same, but various points would change each time you started a new "quest" session.
Basically, as your character progressed through the storyline of a zone, more quests would unlock, allowing you to start at a more advanced point of the game with that character.
So, if you left your current game, and reset the same chapter, or started at a later chapter of your quest line, the map would reset, repopulate, and some new points (dungeons, cellars, crypts, tunnels) could potentially open. It was very similar to Diablo 2 in that respect.
Game works with sets of checkpoints. So you can get to a checkpoint, clear and area, and then if you stay logged in it will remain cleared. However, if you leave the game before reaching the next checkpoint and come back you will be back there with everything repopulated. It is a decent way to farm. You can also just change the quest you are on, but checkpoints are more frequent than this.
