Try another language... I use Scottish or Irish gealic names - goto an online dictionary - if your a bowmam class type in 'ranger' instantly comes up as 'Maor' which sounds is okay and fairly unique ... of course the literal translation of Loach is also 'traffic warden' but meh ... of course more ppl in ireland speak Chinese than Gealic nowadays, and i think only one family on some island somewhere speak Scots Gealic, shame really.
Latin is good too, or Welsh, or basque might work, basically dead or obscure european languages are all good.

This is the kind of process I go through when making a new name:
Usually I take a word, spell it backwards, then tweak it until it sounds "namey"
Also depending on the species/race/type of the character/game genre I might want to throw in some bits that are common to that racial type etc. Refering to books can help
e.g. in J.R.R. Tolkeins "The Hobbit" some of the dwarves have some of the letter groups "olin" "alin" "ili" "oin" "ori" in their names so I might want to squeze one of those in if playing a dwarven character.
Right what about my characters history and background? If I am playing a half-orc then it is likely that my parents used words with few syllables etc, so they probably wont give me a long name, an elf mother on the other hand might choose a longer flowery type name.
Ok, important point... people are going to have to type my name, sometimes quickly, so I don't make it difficult to type, sounds wierd, but if you type a long line of text to me and I dont recieve it because you missed a letter or something, you are only going to try so many times before you give up, also the moment might have passed. This is important if I am going to be spamming for trade or something and want a lot of people talking to me. So short names are good but if I have a longer name, I try and keep the spelling fairly intuative, put a "u" after a "q" etc
I think about the name and make sure it doesn't have any slang or offensive associations or sounds etc I might not want.
Then finally does it sound good to me, can I play under that name for the next 3 months or whatever.
People are very protective of their names, somebody in my guild told me they copied my WoW name on another server.. I chose to be flattered, but I have to admit to the odd twinge of berserker rage and blind fury at their lack of imagination and identity theft hehe. Thats why I didn't use one of my four or so favourate names when creating this account at mmorpg.com, talk about paranoid or what! (I nicked it from a character in Baldurs GateII Shadows of Amn)
EDIT: I just want to say, what you do with your character will decide whether the name is cool or not, well I think so anyway. Names sort of grow on people depending on their how you interact with them, If people like the way you play your character or you yourself, then your characters name will become sort of cool in their eyes. The guildy I mentioned who used my name told me it was a really cool name... but it wasn't really, not really, I think he just liked me and my characters shiney armour etc (he was young in real life so it was kind of cute really)
If you are not a roleplayer, then you probably want to ignore most of the above and choose a name that makes you stand out, be aware of naming conventions in your game though.
Sometimes I put people on my ignore list simply because their name goes against the naming conventions on a roleplay server, I get a bit fed up with it to be honest. Things like "IpWnEvErOnE2Deth" and "DEATHbrinGer" are common in WOW, I always seem to pass their corpses on the road which makes me smile hehehe.

I use foreign languages and a brief description of my character to create names.
For example if I wanted to make a germanic character in a game I would think "ok what exactly is my character?"
If it were a Dwarf Warrior, for example, I simply translate it thus:
Dwarf = Zwerg
Warrior = Krieger
So my Dwarf would now be named: Zwergkrieger or I might shorten it up some to be Zwekrieg or something like that.
This is a pretty dull and plain example. You can be as bland or as creative as you want. Most common names are actually just a combination of words that mean something. Kevin, in Celtic, for example, means "Gentle" "Kind" or "Caring" in Celtic. In other languages it has different meanings. So the EASIEST way to make a name is just think of something you like to think of your character as, then translate it into a foreign language
German works REALLY well for Dwarves, Ogres, Trolls and other "harsh" beings because German is a very hard language with lots of strong sounds in it. French, Latin and Spanish are awesome for elven names. For 'normal' names I just whip out the name book we used to name our kids and start flipping through the odder ones Worst case it's ever taken me was 5 or 6 tries to get one that wasn't in use.

I have Rhapsody on my computer and so I listen to A LOT of music and I actually used a name of a song for my character. And for the next game I buy, I plan on using another name from another song.
In fact Eminem has a song named My name is. And it turns out to be "Slim Shady"! Use that! j/k
