Are we really at a limit when it comes to PvP rulesets of either (2 Faction, 3 Faction, FFA, or no PvP at all) in the MMO game industry?

Why havnt the genre expanded on its PvP options yet?

You can have multiple factions within Free For All rule sets aswell. These factions are usually player created and dev supported like in Age of Wushu and EVE.

There is also Full loot., but you can have that under any of these rule sets.

I'd say scenario PvP should be considered a stand alone PvP concept, other than that, there aren't really any other options that can exist in MMOs for PvP. No PvP is just that, faction based (regardless the number of factions) is a ruleset placed by the game, and FFA allows players to determine their own rules (problem with FFA is the community tends to be too ignorant to create a unique PvP experience and instead it's just a gank fest).

Not sure what other concepts you would propose to developers...

Games that have arena-only PvP often don't have PvP factions.  Wizard 101 is an example; PvP is 2 teams of 1-4 people of any class (aka element).  In the game's story all of the classes are treated as semi-cooperative, semi-competitive factions; there's some backstory about how a 3-way war between ice, lightning, and fire (iirc) exploded the world into it's present-day fragment islands, but those three factions declared a truce after that disaster.  Meanwhile there are also hosilities between dark and light, and maybe between nature and science (again iirc, it's been a few years).
I want to help design and develop a PvE-focused, solo-friendly, sandpark MMO which combines crafting, monster hunting, and story.  So PM me if you are starting one.

Because a great many "designers" in this industry seem to have no imagination of their own.

Or they don't learn from past mistakes.

Or maybe the people financing the games insist they stick with a 'known formula' because they have metrics / data on that and they have projections that can estimate outcomes.

    Pick one.

But yes, it needs to be done.

    It can be done.

Give me $200,000 for a progammer (and myself!) and i will show you an alternate model! ;-)

What are the options you feel are missing?
