In the next major MMORPG, the only way to gain access to an additional character would be to marry and impregnate a female NPC. If you're female, you must marry and be impregnated by a male NPC. Moreover, you will be limited to wearing cloth armor during pregnancy, as standard plate armor will not fit. After 9 days, you will give birth to another character slot.

So a game where men have all the advantages? Dont think that will go over well.
You would have to go to a member of the appropriate class, and have them complete the level for you. This could tie into the next idea...
Experience would be temporary, but levels are permanent.
Experience works like HP/Mana and is used in combat. Once you level, you are locked in (cant go backwards) but until that point, you lose exp when injured, when using spells/skills and even over time (slowly). The only way to advance is to push to that next level, then lock it in quick.
A MMO where all raiding is entire based on dance routines, the boss dj's play ever more complex mixes, and the raid have to synchronise their dance steps. Rather than dying,
if you fall out of step a npc walks over and awards you with a cabbage that your char has to carry which causes you to burp (it's hilarious always ofc)
If you do really well you get to attach a pompom to your shoulders (well animated). The best dancers have pink glowing pompoms.
Sometimes a narrator will declare 'niiiice' if you are particularly sharp.
rpg/mmorg history: Dun Darach>Bloodwych>Bards Tale 1-3>Eye of the beholder > Might and Magic 2,3,5 > FFVII> Baldur's Gate 1, 2 > Planescape Torment >Morrowind > WOW > oblivion > LOTR > Guild Wars (1900hrs elementalist) Vanguard. > GW2(1000 elementalist), Wildstar
Object: Pick one of 6 Food Characters - Hot Dog, Pizza, Hamburger, CornDog, Bacon, Cupcake and battle your way across an expansive Restaurant with thousands of other players for power ups such as new toppings, condiments, and utensils until you finish the end game raid content where you finally fulfill your purpose. Upon beating the final raid, you roll randomly for the chance to be eaten by the NPC boss. The final prize for being eaten is Permadeath.

Solitaire the MMO: Featuring intense GvG battles where your guild can capture enemy castles or defend your own by guild members winning more solitaire games within set time than the opponent guild.
Quess When to Log Online: A game where you initiate a fight, then go offline. You'll have to guess when is correct time to log back online (random time within next 2 hours) in order to win the battle. If you log at wrong time, you'll lose.
Raids are done by whole raid going offline and then doing a synchronized login back.
You use the extensive character creation system and 6 hours later when you're done, the character goes online by itself and plays.
Then you get a video clip with weirdly inappropriate obscure random music added, auto-uploaded to youtube for you with the weekly game play highlights.
You level-up according to the number of hits your video gets.

Post Online: Each week the players of this MMO are given a random online forum with aim to make as many posts as possible without getting banned. Levels are then awarded based on number of posts.
PvP battles are always done 1v1 on forums focusing on pregnancy discussion.
The craziest idea will be rewarded the following: An OP edit with your name mentioned in giant pink font!
GO!
Reverse PvE MMORPG
Where you are the creature/mob and the NPC's are humanoids (Human / Elf / dwarf ) and they seek you out in your respective dwellings ( Camps / Caves / Dungeons) depending on what type of "mob" you are. The more "players" you kill or the longer you stay alive your power grows til eventually progress into a boss. If you go unchecked you can ascend into a raid boss where you can influence the area (progress into a Dark Lich Lord can turn the forest where your camp is based into an evil forest etc.)

Simple "MMORPG the MMO". You got a bunch of npcs and Bill is the ultimate boss.
you'd have to enter a dungeon called the forum and when doing so your head would be chopped off and stuck up your ass. You can only get to the final boss after having replaced your head in its correct spot by defending yourself against the mass of PUGs.
Whenever you get stuck in the forum and want to watch a how to on the next step you would be stuck with a 2 minute advertisement for WoW before the informative video.

Lol, you forgot the prenuptial agreement where half of what the spouse has becomes hers... clearly the men are disadvantaged.
I still think the Highlander world would make a great PvP centric MMO. You get one character. Death penalty means that character is gone. Then, you get to start all over with a new character. Remember, there can be only one!

Personally the only modern MMORPG trend that annoys me is the idea that MMOs need to be designed in a way to attract people who don't actually like MMOs. Which to me makes about as much sense as someone trying to figure out a way to get vegetarians to eat at their steakhouse. - FARGIN_WAR
You play as a cockroach that is born within the walls of an apartment complex.
You level by collecting food in dangerous places where you could be spotted by humans. As you level you gains skills like flying, faster speed, stealthiness, resistance to poisons. you could get roach armor with stats like +5 protection from smashing, +2 squishability to fit through smaller cracks or talents like escape death where you immediately teleport out of sight right when you're about to get crushed.

Class advancement would be interdependent.
Fighters would require a Healer to level.
Healers would require a Magic User to level.
Magic Users would require a Rogue to level
Rogues would require a Fighter to level.

simple concept, when you initially creat you character you have 10 minutes to ensure you are at least 100 yards from anyone.
If you are within 100 yards from anyone you explode. Ganked love this game.

And if your character die you have to pay 50$ to be able to play the game again!
