ofc old MMO can surprice us, developers need to return to the basics with ofc making feature update to our time.
Look at Lineage 2 Classic, this is L2 old MMO hardcore as before but with many feature remade well done.
With the new patch coming tomorow in KR (L2 classic Saviors) they succes to bring new and fresh idea from old mechanic
We are waiting innova to open our EU server btw ^^

Once I lose interest in an MMO, it's incredibly hard for me to get back into it. New classes, new abilities, new zones and content. Same gameplay and design that I got bored of and walked away from. But I also feel that 10 years of playing MMOs has warped my sense of fun or good. So many these days just feel like I've played it already. TBH, the most fun I've had gaming this year came from Skysaga.

For some reason Champions Online keeps calling me back. If CO does some serious development and rebranding I really think it would surprise many.
