A lot depends on what you mean by running the game "well". You're basically right at the minimum system requirements now, or possibly slightly below depending on which version of the GTX 460 you have.
On a $200 budget, you're in Radeon R9 380/GeForce GTX 960 territory. As the R9 380 is both faster and cheaper today, getting that over an Nvidia GPU is an easy call. Take your pick, depending on how you feel about rebates:
My general guideline is that, in order to justify upgrading a video card, you need to at least double your performance. That puts you in Radeon R9 370/GeForce GTX 950 territory. As the latter is both cheaper and faster, that's the pick in this price range. You could go that direction if you really want to keep your budget far below the stated $200:
I'd advise against getting the GeForce GTX 750 Ti mentioned above, as it's not fast enough to justify bothering with the upgrade. I'd also advise against getting a GeForce GTX 960, as it's a poor value for the money.
Unless, of course, you're not in the United States and looking at totally different prices where you live, in which case, I'd want to see the prices.

Awesome! That was the card I was recommending to you based on the price you wanted to stay at or under. For the money it really is a great card. I think you will be surprised by the bump in performance.
Yeah thanks a bunch. Your comment allowed me to cool my head a bit and decide on it rather than spending almost 100 more for just a little more performance I don't think I'll really need.
I figure it will last me at least a year, and then I may want something totally different anyway since the new chips are coming out. I should definitely have more income anyway in a year's time so I'll be able to get something nice.
Main goal currently is to be able to play Dark Souls 3 and a bump in BnS performance. 950 should do the trick.
The game is extremely poor optimized. Gfx's running The Witcher 3 on medium / high settings score less than 25 fps in Dark Souls 3. The weird part is that lowering the detail levels even the resolution doesn't have any impact at all in terms of fps gain.
They are rushing patch 1.1 out which, according to the developer, will boost fps by up to 40% at 1080p.
The question is why a high profile title like DS 3 was released at such a poor state in terms of optimization.

From software and bandai namco have a history of poorly optimized pc ports.

Just got my card. Dark Souls 3 looks amazing with it. Not sure about fps but it's smooth on high. With my old card I couldn't turn around on lowest settings.
BnS seems to look a bit better but as an MMO it's more CPU reliant and my CPU is ass (some cheap i3). That's a big reason (besides money) that I didn't want to go too far. Anything I get for MMOs will ultimately be bottlenecked by my CPU. That's my next upgrade, but in the meantime I'm now able to play everything I want so that's what matters.

BnS seems to look a bit better but as an MMO it's more CPU reliant and my CPU is ass (some cheap i3). That's a big reason (besides money) that I didn't want to go too far.
