There are games out there that meet your request, but they lack players. The question is, are you really looking for the above or for a game that actually has people playing it? In this day in age it's either A or B... not A and B. You want to play entirely on your own, the list is long. You want to play with others, the list is almost nonexistent.
Thats probably why i am allways looking for C in a binary world..

However it is no longer the case as just like FFXIV they have turned it into more of a WOW clone,however make no mistake there are a LOT of real tough battles.
However nothing to stop a group of players from playing it vanilla style and you would then see how tough it really was.
I mean in reality questing for xp covers 99% of the genre and that is SUPER EASY mode.VERY few likely you can count them on one hand that give xp via combat only.
Example in FFXI unlike questing where you fight mobs same level,you fight mobs several levels higher,no way to be done solo.Those fights would have to be very difficult to get enough xp ,which has changed a lot now basically 2-5x more xp per kill.
Example your group might be level 25,you would be fighting stuff level 28-33.As soon as mobs are even 1 level higher,they are harder to hit and you do a lot less dmg ,taking a lot more damage.Since game has changed,only those that played it back in the day know how tough it really was.

It's not your traditional theme park MMORPG, as it's basically a lobby and semi-randomly generated instanced dungeons. But it has the best combat I've ever seen in an MMORPG, and it's designed to challenge you, especially once you get to tier 3.

Agreed. FFXI, before Abyssea, is still my favorite MMO of all time. Leveling was challenging and fun, story fights were tough and required team work, end game was diverse with multiple routes to good gear, and there was lots to do besides fight constantly.
To the original poster: challenge is certainly subjective. Perhaps a bit more detail?
Are you looking for a critical thinking challenge? Dungeons and Dragons Online would be a good start, lots of puzzles and mazes to challenge you.
Are you looking for a combative challenge? Very touchy and very subjective. As many games can accomplish this either through timing, reaction, attention to detail, the list can go on (thus subjective). I honestly can't even think of a particular game off the top of my head that was truly challenging in the combat, maybe a couple encounters, but a pretty even field in that department.
My suggestion would be while leveling, don't over-gear and over-level the content? But who likes bandaids anyway?
Hey. I know the feeling. I've been searching hard for a good mmo (or something similar such as a diablo style with multiplayer possibility) with a challenging pve on the whole where leveling is relatively slower (as was typical in the past) plus the mmo has to have at least fairly deep customization for level progression (ex. big skill trees, cross skill/class group picking or something similar). These are the two things that to me have to be there at minimum to make an mmo fun for me. The rest of the usual items for -my- mmo (ex. graphics, story/plot, pve capability, character appearance, combat mechanics, etc) of course should be somewhat decent at least even if one or more of such is not that great (I’m at a point now where I can live with certain weak points among such remaining items and still have fun overall).
