Good Thread so lets start dreaming :P
PvE: I would try to make all ppls happy, 5-10 man dungeon, raid and solo players oh and crafters too :P. Which would mean u are not forced into one gamestyle which you dont like. Sure all should take same effort and be somewhat equally time consuming but the option should be to the player.
PvP: Well I really want PvP with meaning to the world which would mean you could take over towns, castles so your faction would control em then. Maybe not the capital cities from the races you could choose from but at least there should be other ojectives to these cities. There also should be lands which you could take over and build more units (NPCs which would patroul the land where u place em, sure there should be a limit but u also could equip em with better gear for more money and so) to defend the land which costs alot of gold. Also PvP vehicles like catapults and so for castle / town sieges. To build towers for defense well all this type of stuff you also know from RTS games :P.
Combat: Combat must be real fun more action like and not right clicking a mob and then wait until its dead this is boring.
Maybe a Sci-Fi oriented MMO, why always fantasy ? dont get me wrong I like it but I also want to be once in year 2050 or even 3000 with futuristic weapons / armos and fighting mobs u couldnt even dream of. Man that would be cool xD. Maybe make it like in a FPS where u really have to aim for mobs and ppls in PvP but it still would be a MMORPG where armos / weapons / upgrades playing a big role but skill too (well u can screw this idea if most ppls wouldnt like, but still make once a futuristic MMO).
Sure there are alot of more things i could think of but that are the most important to me.

The only race playable would be humans. The timesetting would be undefined, a bit like the 16th century, but more advanced societies, regarding politics and such. The story is kinda an cliche. Once, there was this big nation with a great leader - Although the nation was far from perfect, most was happy. The leader died, and four different factions emerged, each with their own political ideology. This is everything from the use of slaves, to regulation on weapon cariages. The country was split up in huge civil wars, where people who sworn loyality to one faction would seek to one of their controlled cities. This creates a world divided in 4 nations, all in war with each other. But there is also the minor factions, which have their own ideology. This may, or may not be close to one of the greater factions way of thougth. These small factions can swear loyality to an faction, while still retaining much of their freedom - Or they can choose not to, being a kind of independent outlaws.
The player is thrown down right in the middel of this conflict. He can swear loyalty to one of the bigger factions(Ultimately, these factions are not player run. Their leader is an NPC. But there are positions that gives great powers, avaible to the player), or one of the smaller(Which are, player run factions). There is no class system, but an in-depth skill system. PvP is the focus in this game. Defending and attacking rural towns, attacking castles, raiding cities for personal gains, acting as a spy, assasinations of other players(Permadeath, yes. Sounds harsh, but gives the game much more immersion, and makes the world totally dynamic), thieving and so on. Or maybe you want to be a rich merchant, controlling your own mine and army of blacksmiths(Which, except for a very few NPCs that makes basic tools, are all players)? Or maybe you would just be an unnamed traveler going from town to town, joining up with anyone, as long as there is gold to be found? The setting is pretty low magic. Magic items are indeed very rare, even the king of a nation have only 1, maybe 2. This makes steel way more valueable, more valueable than gold even. The focus is good craftmanship weapons, not a +1 sword. Magicians exist, but most are only able to cast petty magic. Petty magic can though, indeed, be quite usefull. Think Warhammer Fantasy Roleplaying Game. If you're lucky, you might be able to join the great magician academy - And either become a full fledged wizard, or die trying.
One of the factions have regulations on weapon carriage amongst civilians - This is a nation where martial arts have been commonly trained for the last century.
The gameplay will be twitch based, making it hard to master. Anyone can pick up a sword, but mastering fancy sword play is an entirely different story. Depending on how and where you hit, people may go down with only one blow.
Each faction carries traits from real world countries, but non of them are an complete copy. Rather, it's a mix of interresting cultures. One nation might be a mix of ancient greece, and England in the 1700th century.
