I'd say anything that makes me realize I'm being cynically manipulated into the subscription cost. Naturally, this awareness can't be completely escaped.
The point at which the tedium of the grind becomes greater than the enjoyment extracted is the kind of thing I'm thinking of. Making sure I play longer by repeating the same task five thousand times doesn't work on me. I just cancel. Alternatively, real-time and pay-for-skill/item systems whisper the promise that if you but pay for a little longer, you will indeed become more powerful. Your involvement in the process is not needed, merely your funds.
Also, excessively revealing armors are all but hitting you over the head with a baseball bat inscribed with the words "WE ARE TRYING TO SELL THIS PRODUCT WITH B00BIES!" Damned if they'll pull me into a deal by my crotch.
I would say the HUD (heads up display). In some games, (Guild Wars and World of Warcraft) you have shortcuts that alow you to press a button and BAM! Your casting a spell or attacking an enemy. I think it is anoying to have to click through menus all the time in other games and have to cast spells that way.

usually it just becomes a nerf-fest cause it is easier to take something you already can do out of a game than to add something to it
