DDO is 100% Instance Grind with some Questing thrown in.  No world to explore.

WoW is Quest + Group + Grind.

    Quite fun.

New kid on the block is Lord of the Rings Online.

So far, very heavy on Questing for fastest advancement.

A good MMORPG with much less focus on Questing in Anarchy Online.

And, you can download and play the full base game for as long as you want for free.

Only if you want to play the expansions do you need to purchase and pay a monthly.

You are gonna have a hard time finding a non quest MMO right now but still some out there.  I think EQ2 is a pretty good non quest MMO since you can do dungeons without quests and get great exp & items in that manner.  DDO is 100% quest/instance driven with some new outdoor type areas, it is also designed to be nearly 100% grouping.  It does have the most exciting gameplay in MMO's at the moment which is why I'm playing it.

I love DDO, but if you hate quests than it is definitely not for you. They are the best quests I've seen in my experience with half a dozen MMOs, but that's pretty much all there is to really level up with at this time.

Personally, I hate wandering around generic terrain hacking at the same monsters with different skins on them. I also dislike dealing with other players whose idea of fun seems to be to stop you from completing your quests even though they are supposedly on the same side as you.

Shoal is right on...or 'spot on' if you prefer lol

Played AO from Closed beta thru Shadowlands expansion then kinda burned out like all the WoW folks are experiencing now, myself included. Created 2 free AO accounts and am playing again. It is a great change of pace, worth a look at least.
