help me find a new game

So I have just come out of playing WoW and SWG for like a year and a half and I am looking for a new game.  Can you recommend a game from my list of features I am seeking...

required...  

good combat sytem

different factions and PvP (like WoW alliance vs. horde)

elaborate crafting system

good graphics

medieval, elvelike fantasy setting with swords and armor etc.

guilds

and just a fun game in general

Thing I would like but not required...

guild housing

doesnt take loss of sleep, job, social life, family to get into the high levels
