I wonder if you have the FakeFullScreen issue...I did..it was pretty horrible but I found out about it last night and the game is pretty smooth now at all maxed settings in 1600X1200 for me.
If you have a LCD and a fairly high end graphics card I think it may be that problem..try this

You have to enable Vsync but it doesn't work (its a bug). To be able to turn it on when you check the Vsync box, you have to access your .ini file. Its in My Documents/Lord of the Rings Online and its called UserPreferences. You can open this file with Wordpad (maybe its another name, I have the french version of Windows). In there, you will find the line AllowFakeFullScreen=True. Remove True and put False instead. Then save this file, quit and run the game. Access the graphical options and enable Vsync. Thats it.

big differance between closed beta and live is several thousand more players on the server, no idea on network load or anything but at a guess all those extra thousands of players connected must make a differance to bandwidth etc
more players in area means a lot more textures to load, plus did u install the extra textures pack ?(which was released at pre-order stage) i know that made me to have to tweak my settings a bit
Adv graphics, near the very bottom..they didnt call it VSync though for whatever reason..I think its lock refresh rate or something like that? Its a clicky at the bottom of the advanced graphics page.
Going to try that fix when i get home. I dont think the choppy stuttering is anything to do witht eh amount of payers. I recently rolled on the new server and even in the wee hours of the moorning where i might not see another player for a couple of hours its still the same. Ive even turned the gfx down to medium and this still doesnt help.
Greatness is difficult to appreciate from close up. The great mountain on the horizon is only the ground when you are standing on it.
The only lace I get any hitching is in Bree or the Forsaken Inn during peak hours. If I am going to be spending any time in Bree I just kick my graphics down a to medium then go back to ultra high once I leave town. It's the loading of lots of different character textures that causes it for me. I prefer that though over playing a game on empty servers or a game where everyone looks the same. I figure that is just the price for playing a popular game that has a lot of different armor andgear.
