Rated M doesnt mean adult only can buy it.

M rating means major stores like wal-mart , best buy , K-Mart , and such wont carry it as a stock item. It possible to buy any rated M from a speciality store with nothing more than a pre-paid visa or even simplier Amazon.com account connected to a family account.

Most rated M games dies , or become banned in some country, thus making them either imports or high vaule on ebay. Ps2 , the PC and Xbox 360 has a ton of small produced M titles mostly soft-core porn related. Most of them are banned in Euro , and few a banned in New York , Texas and Rhode Island.

The most famous case of a MMORPG rated M was Priest Online , which was shutdown the US side after intenst pressure from varies groups. Most game companies aim for Teen range to appeal for mass appeal and the ability to showcase it.

There is no way to prevent anyone from buying anything if they really want it.

You are thinking AO (Adult Only) my friend.  M games (at least where I am at, Midwest USA) aren't that hard to come by.

Edit:  To answer the OP, it doesn't really mean much.  Some retailers may ask someone in question to provide identity to show they are at least 17.  A year or two ago when I bought Resident Evil: 4 the guy at Best Buys actually wanted some identification.  I simply called my dad over and had him buy it.  This is how most of the "kiddies" people are worried about can get by the rating.  I am not aware if the rating is just a suggestion for parents or actually enforced by law in some parts of the world.  The "M" rating doesn't get as much flak and blockage as the AO games do.
