My PC is towards the higher end, I built it a few months ago based on the system requirements of Tabula Rasa.
I picked up both EQ2 and Vanguard "for the helluvit" last week and added them to my Station Pass. In Vanguard, a new game whose performance issues and complaints are near legendary at this stage, I'm averaging 40fps in Fraps with my graphics settings fairly high. However, in EQ2, a game that's been out for 2.5 years, optimized and such, I'm getting 20fps with settings somewhat comparable to what I have in VG. (Not fully maxed in EQ2 but trying to make it look good) Honestly, it's just not enjoyable when things jerk that much.
Suggestions on settings that truly impact performance without taking a huge hit on the quality?

There could be multiple reasons, but I think it'd be nice for others who may take a guess if you post your system specifications, such as video card name and model number. Keep in mind both games also use different graphics engines, and from what I have seen one is optimized better for ATI while the other for Nvidia.
You may notice the Nvidia logo here and there with the install of EQ2. I have not played Vanguard but if it uses the Unreal engine, I know I have always had good luck with that engine on my ATI cards.
So I may be taking a bad guess but do you have a ATI card in your rig?
lol, why would SOE "optimize and such"? There is no motivation todo so. No return on investment. Dev's dont do that kinda stuff anymore, that was left behind in the nineties. Microsoft is in the same boat, just look at how crappy Vista runs performance wise vs XP. They get paid to work on the next expansion, not "optimize", sheesh.

This game has always had system performance issues. After almost 3 years, SOE still hasn't fixed it, so I wouldn't hold my breath.
It is NOT just me!
First, I love EQ 2. I wish I had discovered this great game earlier. I wasted A LOT of time in WoW.
Second, my performance is sometimes worse in EQ 2. I lag more. I never have crashed. I never had some of the problems I had in Vanguard but I do lag more in EQ 2. On my server, Antonin Bayle, the population (unlike many servers) is very healthful and vibrant, especially with all the new players starting. So, I think that must be it. Anyway, it is ironic that I sometimes lag more in EQ 2.

WoW and fast food = commercial successes.
I neither play WoW nor eat fast food.
I found an article on TenTonHammer dealing with EQ2's options and performance. It's an older article but was very helpful nontheless. I'm up to 40fps now at least. Shadows were the primary culprit, unfortunately.
Now, if the mob graphics were better... I really, really, really hope it's just the noob zone mobs but the ones outside Qeynos look like shiny playdough stop-animation models, not living breathing creatures.
Go into options and turn off cloth simulation if its checked a real system hog that one and for what it does its not worth it.
try put performance settings on 'High performance' then put textures, rendering distance and character textures to the max, it works smooth like that for me, and still looks awsome.
I'm really frustrated with EQ2 performance. I have a pretty decent PC (3.2 Ghz Pentium 4 Processor, 2 gb RAM, ATI Radeon x600 256 MB graphics card) and my graphics card locks up when I attempt to make the graphics look decent. To get it to run and not lock up, I have to make everything look blurry and dull, its to the point that I won't play EQ2. I tried the newest drivers at the time and that didn't help, any suggestions?

I have an Athlon 64 3ghz, with 1.5 g ram (burned out .5 recently and haven't replaced it yet), and an ATI x1600 (512mb) video card. I run on balanced most of the time, and performance when raiding. Runs smooth as silk, and I didn't even notice the .5 gb in ram decrease. Some of the UI's from eq2interface have custom graphic settings already built in with one click of the mouse, so my options always says custom, but those are the closest approximations. Use WindowsXP SP2.
EDIT: BTW, I have screenshots of what it looks like in game on this forum to me with my settings, so you can obviously see it looks great with my settings and still runs awesome.
