I am wondering after having reached level 5 without being killed and was given the wary deed, and then
killed in battle, can I still achive more deeds for not being killed as well?
Also is their a way to repair my weapons and armor myself without having to go to a merchant or trainer
to get them fixed?

I think once you've died that's all the deeds from not being killed you will ever get.
No I don't believe you can repair armor yourself, this is a way to create a money sink or part of the death penalty. I think ^^
(,,,)=^__^=(,,,)
You can repair your equipment yourself by using repair anvils. But these are high-end items (need to be a master crafter and have high standing with certain factions), and using the anvils will be even more pricey than repairing at a normal NPC vendor.
Where the anvils come in is repair on the battlefield (i.e. not going back to civilization), and while not cheaper for the crafter, it is for his fellowship, whose armor he can repair.

The final title you receive at level 20 is The Undying.
Its debatable whether or not the Undying title is worth a lot. Mind you it is a status title, it doesnt do anything. Also it must be noted, no one dies in Lotro. Your simply disabled and your morale is diminished you can no longer fight. For storyline purposes all defeats are near misses. Since a death would spell the end of your career. Rather the title is a symbol your one tough cookie and never had a near miss. Personally if Im defeated Ill start over and try again since its just a personal goal of mine
