number of newly activated accounts is still growing at a rate of over 200,000 a quarter even after nearly 4 years since being released.

ArenaNet deserve every one of those sales. Roll on 6 million!
It's interesting that the release of Nightfall and GWEN correspond to relatively small spikes in the account numbers. If counting every campaign or expansion separately, I'd expect to see much larger spikes in the numbers than that. I had implicitly assumed that that's how they counted things, to inflate the numbers. Perhaps they really do mean new accounts, and if someone adds more campaigns later, that doesn't get counted again?
