Does any MMO still do this?
(I'd like to see WoW try this with its 600 copy and paste realms...)

I am fairly sure that a small-scale MMO still could do this but as you already acknowledge it is just not practical for any large scale MMO do to in a manner that is not just a one-time gimmick.
Ryzom does that.
In december they had an event that had the entire server following a NPC into a pvp zone on some three part quest.
It was pretty cool actually.
