Back in the day when I had 12sky and played it, I seen only some few players with plus 11 gear and I was temped to research on how to get like them. So my reaserch lead me to this one of the most powerful buff in game from a skill book called Garudo Song, a skill book for boosting your luck very high. So I was also told that players would flock to the ones that had this luck skill and would pay them to enchant and plus there gear because of the high luck. They had great luck in making gold from other players even if they failed with all that luck from the buff, players did not even get mad they just keep coming back I was told. So I was like I want this skill book how can I get it I ask and so I was told you can buy it off someone for 180mil gold or you can get it as a drop from mobs and that can take you a very very long time because of the rarity they told me. So at level 85 I got that lovely book to drop in some cave I was leveling up in, I got so excited I nearly fell out of my chair! So I shouted in game, a mistake, due to being in shock "Garudo Song book just drop!!!" and so for that I received a total of 45 offering whispers, the highest offer was 230mil gold!!!!

Probably rarest item/s I've ever gotten are just some raid epics when I used to play WoW, nothing fancy really. Oh and some oozeling pet that was pretty cool.

But okay, I'll bite. The rarest pair of items I received (as far as I know) were an Aurilian Banner and a Sunrider's Destiny lightsaber crystal (among other things) in Star Wars Galaxies right after the NGE went live, when a Jedi friend of mine quit the game for good.
They didn't do me much good though, since I believe I just left them in my house when I quit less than a week later, heh. Funnily enough, this demonstrates my point about the fact that the items themselves ultimately aren't important.

For me it would be unlocking Jedi in 2003 and getting my Jetpack, even though the Jetpack had no animation getting on it and you looked like a plank of wood when flying.
Oh man, I wanted a Jetpack so bad. I must have killed about a thousand Black Suns (like ~50 per day on and off) with no Jetpack Base drop before I got exasperated. Couldn't afford to buy one either.
I did get a Black Sun Ace Fighter Helmet in space though, which was pretty sweet.

I had an unlocked jedi also in SWG and had 3 nightsister biceps, 2 I gave away in pre cu to friends, and 1 I kept for myself.
When I played WoW, there was a small baby gorilla pet you could only get through the TCG, and it was supposedly rare. I think the TCG is a waste of money, but I really wanted that pet. So I bought three packs of cards and found the card I needed!

A personal favorite for me was the [Hanzo Sword]. I know it's pretty lame overall, but it was a cool thing for WoW to have put into the game.

If you don't know what that is; it's a one-time only reward from a server event to open the gates of Ahn'Qiraj in Vanilla WoW and therefore make it possible for everyone on the server to do the raid instances behind the gates. The guild I was in had the honour of opening the gates on our server waaay back in 2006.

Threads such as these will lose their meaning once the RMT happy crowd starts dominating the next generation of subscription games. It is my hope that that particular model will fail, because the sense of achievement (no matter how small), will be lost for good when everything can be bought and sold with a credit card.
I don't know if anyone will remember it but the first version of the rebel theme park questline in SWG had a "endor" style helmet as a quest reward. They changed the reward soon after and there was no other way to get one.

hmmm all would have come from EQOA. for some reason it was the only game i played that had actual rare items that you may never get...ever.
umm first thing would have been an "Arcanists Robe" it may not have been as rare as it seemed. but it had HORRIBLE stats like 2 resist stats and not much of either. so if people did get it more than likely just junked it.but i'm a bit vein and i always try stuff on no matter how crappy the stats are and equip didn't bind at all in that game so didn't matter. but the robe was a completely unique color wich was a super deep dark blue. i would always get tells asking to buy it or where i got it. later tho in the expansion they added couple of robes with that color and minus the suck stats so meh.
the other in the same game was "2 horned Ripper" another item i believe was made rare was the fact that its icon in the bag looked like a meat cleaver,and again lame stats, caster only. was out exping with others and it dropped no one wanted but i picked it up to junk later. for what ever reason i tried it on for shits and it ended up being a 2 handed sword. THAT got me crazy tells asking where i got wanting to buy ect ect. eventually they made it no longer drop in game at all because "casters wasn't ment to carry 2 handed swords".
neither of those 2 items did i try for, was just plain dumb luck and a neat bonus. 1 item i went out on a mission to aquire was the "Crystaline Orb" it was a caster only shield made of pulsing light. me and a friend found out where it dropped some how, and prior to this no one seemed to know and only seen maybe 3 in game on our entire server. Tak Frostbiter a rare named spawn in one of the goblin camps up twards halas dropped it. he had 3 or 4 spwn points in the camp and we had to kill the place holders over and over and over for him to spawn and when he did spawn he almost never dropped it. we farmed for i bet 10 hours a day over the course of 4 days before he finally gave it up. then being the glutton for punishment we was we farmed 2 more over the next 2 weeks. then it got leaked where it dropped and the place ended up being camped 24/7..lol
but deffinitly the Crystaline Orb i got in everquest online adventures was the rarest and felt like a big accomplishment when i got it.even if couple months after i got it alot of people had it

Since I'm assuming it can be any time of MMO, my rarest item I received was the Valor Citation from Killzone 2. I earned it by being part of the 10% most top elite players overall that week. And it's noteworthy that I received this honor only one month after buying (and first playing) the game.

Although not a mmo, my rarest find was a windforce in D2. Sad thing was a massive duping spree broke out the next day and I had to settle for selling it for $100 rather than the $500 it was worth when I found it.
I also consider my staff of theshadow flame (from nefarian) relatively rare from WoW, but the reality is that nothing's very rare in wow, or at least not when I last played it. Darn commie game imo.
