In order to get a hotmail account without numbers I randomly typed letters and got "hrt", I then put in some vowels to make "haratu"... and have stuck with it for 10 years now.
FFXI was my first MMO and I was trying to pick a name I like (which takes forever), so I decided to look through some names lists online. I ended up finding Takai in a Japanese baby names list. I really like the name and I've stuck with it since.
If Takai is unavailable I usually try to go with Tavadon (which is a surprisingly uncommon name in MMO's). I got that name from the book "The Outstretched Shadow".
Currently on a quest to try every free-to-play MMORPG. Okay, maybe not every single one, but most of them.
I usually sit around going "Uhhhhh ummmmm errrrr" for like 2 hours before I create a new name for any given one of my many many characters across all my games. I rarely use the same one, people in game tend to just call me Kat since it's short for my real name. That is the only really consistent name i've kept over the years and the reason I have it is pretty obvious XD
This is actually my first name ever created, but spelt Whisperwind from the D & D Player's Handbook back in the early 90's. Taken from a spell 'Whisperwing Wind'.
Most of the mmos since I've used it, until WoW took it for one of their NPCs Tyrande Whisperwind. I'm not saying they stole it, that'd be a little stupid as with the amount of people out thinking on names, many are bound to be duplicated. However it did put a bit of a damper on my initial enthusiasm of my first character.
I made up the name Solistus for an Elf character in a D&D game back when I was maybe 9 or 10. I got a D&D Elves Handbook and the section on naming and language gave some guidelines and examples - I can't remember what it said exactly, but my end result was Solistus. Friends and clanmates shortened it to Sol or Soli. I use Soli in a lot of games, now; it's shorter, still available most of the time (surprising given how short it is) and easier to remember and pronounce than Solistus (which is pronounced soh-LIST-iss, btw :P ).
I would normally use the name Leynora, Lyonissa or Leyoran for my character names. It's a play on Leyna, a name I got from an evil name generator. I never use the name Leyna anymore though, I thought the above variations are much nicer, and anyway, I've always prefered three-syllable names for my characters. Sounds more grand that way.
There are some games though, that force me to use more than one word as a character name, so I normally stick the name Chia on as a last name. Rather uncreative of me; it's actually my surname in real life.

Umm Well Mialily always seemed to sound like a elf name to me =P so it stuck im always a elf in all games ^.~

My other name Ggót is also kind of historical, based on the name of a brutal ancient german tribe, the goths.
