I would have to say EVE. It's not totally my kind of game with all that steep learning curve and stuff, but i have friends playing it, so it's alright. But i have to say.. that game is deep!
The word "polish" is getting thrown around so much these days, and most people haven't a clue what it means. it's just a term for going over content areas with a fine-toothed comb, and directly relates to the initial areas in a game - as that is where all the effort goes these days. If you want a game that just "feels full", play original Everquest - tons of shit to do by now.

I had alot of problems when i played the game and in general it didn't feel polished at all if you ask me. And yes i would also add EVE with wow on the list there, was a little skeptical first but after thinking about it i believe it deserves it.

I'm surprised at the popularity of Eve in this post. I run into bugs every single day with that game from graphics glitches to gameplay. Also the UI could use some improvement.
On what grounds can you say "most polished"? Guild Wars was the most polished during launch.

Definitely not Eve. It's a huge pile of mess with a lot of abandoned features and crappy code. Battles don't work well and most of the pvp resolves around trying to make it more playable for you and force the enemy into lag.
I'd say GuildWars. It was pretty finished on launch. Prophecies was already working well and most content was in. I never encountered serious glitches or quest bugs and balance was about right. The visual style is very consistent and it didn't really age that bad.
Most of the time it would be the game on the market the longest with the best company. I guess currently that would be EQ2 and WoW. Out of the more recent releases, it would most assuredly be Lotro, as it had easily the most fluid launch I've ever seen...with few bugs...even today there are very few.
I think the OP is not asking for the game you like best but more which is most polished and i played quite few mmorpg and eve certainly is not as polished as WoW or LOTRO.
WoW. Because polish is not really something you can see, it's more the absence of those things wich aren't polished. And in WoW it's rare to encounter anything wich isn't polished, whereas even in good games like Lotro getting troubled by the lack of polish is common.

Um, I love playing EVE, but I couldn't describe it as "polished" and keep a straight face. "Endearingly quirky" would be about the best I could do.
Some people have speculated the the game's incomplete, inconsistent and occasionally plain inaccurate documentation is a deliberate choice by CCP to encourage players to communicate and work together.

I have been flying a ship setup on my guardians for a while that, if you look at the bonuses of the ship, shouldn't be possible at all. It makes for funny kills because nobody can expect that ship to behave in the way I'm flying it, since it's not documented anywhere.

If you consider from launch and continuing from there on, I'd probably say LOTRO.
They had a bloat/fragmentation (dev's label) issue causing crashing but they fixed that with the content release of Evendim. (way before its first expansion)
It was pretty stable in beta too and lacked the bloat/fragmentation issue at that time.
It still hitched when I played it but I left before the first expansion.
WoW still had server problems last time I attended a raid there but that was in the days before the burning crusade expansion. The server plopped when we were at 2% or 4% on Hakkar.
