1. Raidmode. Raiding is fun but it should be a game in and of itself. It forces too many balancing issues for pve that throw non raiders to the curb in terms of difficulty.
2. Forced grouping/catagorizing mobs. If i click on a mob and it tells me solo/group/raid difficulty....blah. Pre eq1 raidmode did it right. All mobs (save for some named ones) were created roughly equal. The benefit and desire to group was formed on A)safety in numbers B)faster rate of xp gain C)xp bonus for being in group.
3. Quest based leveling. Questing is good and important, but should not be the only viable way to level. Some people just like going out into the field and bringing the pain to hundreds of mobs.
4. solo quest lines that all of a sudden find you at a brick wall requiring a group.....
5. john smedly, destroyer of fun and truth.
6. Subscription based models that also have item shops. fish or cut bait, poop or get off the pot. Pick ONE and roll with it....
7. Islands that float in the air....
8. Different dungeons that use the exact copy of another dungeon from the game......
Im with you on that one, if there is one thing Ive learned from playing MMOs and conversing with its communities is that the people are never, ever happy with any of the games that come out
I care about instances. They change the entire game in a fairly big way most of the time. I don't enjoy playing a multiplayer game by myself. I want a virtual world. If they could do it in 2001 they can do it now.
As for gamers never being happy... there used to be plenty of happy people, back when this genre wasn't dead in the water.

It isolates players from the game "world". It takes the massive out of MMO if people are constantly in instanced zones.
If I wanted to play an online multiplayer game I would pick up any other game that doesn't come with a monthly fee.
Oh and phasing is just another example of instancing that is hardly revolutionary.

I try to stay open minded when it comes to MMO features, and have found entertainment in a wide variety of games. That being said, the one thing I can't stand in MMOs is the gear-on-a-stick progression system.
Instancing - some instancing is ok but today's devs seem to think it's all or nothing.
The idea that everyone has to be THE hero - I'll settle for being an artisan or a doctor.
Battlegrounds/scenarios - under the right circumstances I do enjoy them, but they generally kill any chance of meaningful world PvP. I LOVE MY WORLD PVP.
Lack of character progression once you hit max level. Every game should have some sort of Alternate advancement.
Quests that give you better rewards than actually killing boss mobs.
Quests that give you better rewards than crafted items.
Broken economies because gear never wears out and crafted items are less valuable than quest/PvE rewards.
Raid lockout timers - If a game must have instancing I should be able to run them as often as I like, not be forced to play an alt or logoff.
Lack of fun, challenging content at every level of the game. Why do I have to suffer thru 79 levels before I can start enjoying dungeons and raids? My all time favorite raid is still SWG launch day we had 2-20 man groups killing krayt dragons and it was truly epic.
10 man raids. Groups used to support 20 players so how can 10 be a raid?
Level requirements for crafting - why do I have to be a lvl 20 or 40 before I start enjoying crafting?
PvE raiding for PvP gear or AFK farming BGs for PvP gear.
Being rewarded for losing.
Lack of customizable player housing/cities. Instanced housing or plots of land with set points for housing is not customizable.
People who play MULTIplayer games then whine because they have to make an effort to get along with others.
People who play PvP games then cry because they got PvPee'd on.
People who want the best Raid gear but dont want to raid.
People who whine about kids playing their game then turn around and act less mature than my 16 year old son.

Basically, anything that forces me to allow my gaming experience to depend on people that are not my friends. The only reason my wife and I honestly play these games is that they are persistant worlds on a very large scale. We have a few friends we like to play with, but if those people lived in town we'd all be having a game night and playing a good tabletop together instead.
Beyond all that my biggest gripe is when handcrafted items don't outshine nonsense loot drops.
-the same starting area when you play a new race/class -when you have 4 different say realms -EQ2's race selection- think it 'pawns' the experience -3 hour dull instances -certain art styles of some (of course will appeal some people, not my tea though) -the lack of creativity in server choices (PvP, PvE, RP...)

Its a list of common bitching i hate, not what I hate about the game
1. Inventory and item management. (WoW, I'm looking at you)
Eventually your bags are full, you have to do something with all that stuff. Go back to town. What do I sell, what do I destroy, what do I equip, what do I put in the bank, what do I put on the auction house, what do I salvage, what do I use for crafting, what do I send my friends etc etc etc. It's just tedious and boring. I'd just like a better more efficient way to take care of all that.
2. The overwhelming feeling of having too many skills and abilities and how to use them all the best.
Eventually around level 20 to 30, you start getting so many skills, abilities, you inevitably run out of room on your quickbar for it all. It's at that point I start getting overwelmed and start to feel like I'm not using all of it to it's best potential. What do I keep on my quickbar, what do I set hotkeys for, what do I not need to use anymore etc.
3. Gearscore
(Yes, I know my gear sucks, now everybody else knows too! lol)
4. Raiding
I just don't have time for a lot of that nonsense... the gear grind... the epeens etc.
I just like simplicity in my games. Not easy games, but simplicity. Maybe these are some of the reasons I like Mario games so much. It's the gameplay I like, not all the micromanagement.

1. Mounts - Yeah they look cool but I prefer fast travel or map travel as many would call it. Taking an hour to get from point A to point B is annoying and really old school of thought.

Like skill based caracters or no healing or out of combat healing.. or just plainly no magic.. You would still get people to draw fire and others to deal damage or to other things.. but you wouldnt be forced into the same pattern every time. Take a lesson from FPS games here.. you might someone who prefers to rush the enemy and draw their fire and you might have someone who prefers sniper rifle... then you have someone who focuses one capturing victory points or planting the bomb.. and then you have someone who makes sure to get grenades.. but if one guy has to leave for dinner the rest can change their plan or get a new guy who might not fit the hole in the team but then they just figure out who does what from what they got.. Besides even though casters are most often OP in MMOs they also feel weak... I mean if you have control of magic its pretty lame how it isnt better than melee.. kills people just as fast, when it should be awesome and be atleast 20 times better.. would be interesting to see someone make a demo of a game where it took 5 minutes to cast a spell and if you didnt stop him you and your team was dead.. yeah doesnt seem fair if the caster is a player too.. also pretty boring to wave your hands for 5 minutes...
Nothing like going into town only to find several dozen inactive players sitting around trying to sell there crap. Especially when it keeps your from finding/getting to important NPCs. I also hate seeing chat spam with people buying and selling crap. I guess i just love Auction Houses (seems to be the only solution)
Zerging
I'm one of those players that likes to do the missions thouroughly, and take the time to understand why I am doing them. I don't always like to RP, but i definitely enjoy playing with those players more.
Insta-travel
I hate it. More importantly, I hate when the game world is so bland I end up insta-traveling anyway.
Meaningless PvP
Call me a hippy if you want, but I'm more interested in what me and some fellow gamers can accomplish together instead of meaninglessly bashing each other's heads. If the PvP has something to do with the story then I can swallow it (if its implemented well), otherwise, its just dumb.
Endgame
There shouldn't even be a such thing in MMOs period.
Quest systems that assume you've done nothing before accepting the quest.
If I just waded through a mass of grunts for some other quest, and happened to kill Bob the Mega Grunt in the process, it's really stupid to go turn in the "kill 20 grunts" quest and have them go "Oh yeah, you need to kill Bob too!". I should be able to say "Yeah, did that." and not have to go do it again.
There's no reason the game can't keep track of every single thing you do. Many things are already tracked for various kinds of achievements. More is tracked for the devs to use in balancing the game. Just track it all, so if I've already met the objectives I don't have to repeat the same procedure.
Trolls who make it their life goal to ruin a games community so that nobody will start playing the game. Even when the game is fun to a lot of people these trolls deter anyone on forums from trying them. That is what I hate in MMO's.
So after reading to first 2 and the last 7, 8 of this post it feels like ppl are complianing that the game doesn't give them all the apples or the game hands them all the apples too quickly... i'm gonna presume the ones that don't get that apples are younger gamers and the ones that do are the older...so with that you have to look at it at the compainies view...which do I want to please the young or the older gamers...aka wow againest something like EVE.. lets say... now we all know who the money bank here is, which is why every single New MMO that comes up is based of the most succesful.....the ones that wanna take risk will always be in the eyes of older gamers which in the end will end up pleasing the younger gamers... how its always been and how always "might" always be....till of course the next and greatest conquers the leading success which we hope for....my 2 pennies in the pot. =D

Well I hate a BIG number of things, I'm sure to miss some, but here it comes
Lack of challenge. There can be too challenging tasks, but thats actually rare.
Lack of complexity. Too simple rulesystems just wont work with me.
Anything but P2P. Only exception would of course be a truely free game, but that ones not possible as long as we have capitalism. Guild Wars system would qualify too but that game sucks.
Instancing. I want the illusion that I'm playing in a huge world with many other players. Instances kill that illusion.
Racial Wars. I want to join a faction and fight other factions. I do NOT want to be forced to fight all Highelves just because I fancy the Darkelf race. Also I want to be able to play a Highelf even if I already have a Darkelf on that server. The factions fighting each other should be player created (guilds / alliances).
Braindead PvP rules. I like the idea of PvP, but it should be competitive and sportive. Not "oh see that guy just killed a mob lets PK him as long as his mana and health are down". I want peace zones, a special PvP mode (both opponents are healed to full at start and nobody else can interfere), reasonable rewards for a successful kill (like a leveldependent fixed amount of money), a playerkiller system (like PKs get marked and cant enter civilized areas without getting attacked by guards), and so on.
Passive Healers. Yes I'm a "Healer" player. I endured being a healbot in Lineage 2, but after playing Vanguard, this game just killed the idea of the usual passive healers for me. In Vanguard, to be an effective healer, you have to stand at the frontline and use your skills. There was no point at all in sitting down - mana regeneration wouldnt increase this way at all. On the contrary, you have to use your skills to keep mana up. Also, Healers where great solists in Vanguard, too.
Lack of good quests. I dont mind a grind once in a while, but there should be some real long and extensive quests in the game as well.
Endgame. Even worse if its the "real game" and what you did before was just some kind of extended tutorial.
Bad crafting. When someone told me what you can "craft" in GW, I went all "... what the heck ?" I wasnt happy with the crafting options I had in other games, but what you get in GW is just plain silly stupid three times over.
Bad broker. I want a worldwide broker with a good search option. Nothing else will work. Give it to me. Thank you !
Pointless timekillers. Like extreme times needed for travel. I dont need an "i win" button, but I definitely dont see stuff like traveling as an interesting task, and enduring it as an archievement.
No seamless world. For a MMO, I really want a seamless world to help the illusion.
Too small world. I absolutely loved the huge gameworld of Vanguard. Was the main reason I started playing that game in the first place.

Racial Wars. I want to join a faction and fight other factions. I do NOT want to be forced to fight all Highelves just because I fancy the Darkelf race. Also I want to be able to play a Highelf even if I already have a Darkelf on that server. The factions fighting each other should be player created (guilds / alliances).
Braindead PvP rules. I like the idea of PvP, but it should be competitive and sportive. Not "oh see that guy just killed a mob lets PK him as long as his mana and health are down". I want peace zones, a special PvP mode (both opponents are healed to full at start and nobody else can interfere), reasonable rewards for a successful kill (like a leveldependent fixed amount of money), a playerkiller system (like PKs get marked and cant enter civilized areas without getting attacked by guards), and so on.
Passive Healers. Yes I'm a "Healer" player. I endured being a healbot in Lineage 2, but after playing Vanguard, this game just killed the idea of the usual passive healers for me. In Vanguard, to be an effective healer, you have to stand at the frontline and use your skills. There was no point at all in sitting down - mana regeneration wouldnt increase this way at all. On the contrary, you have to use your skills to keep mana up. Also, Healers where great solists in Vanguard, too.
Lack of good quests. I dont mind a grind once in a while, but there should be some real long and extensive quests in the game as well.
Endgame. Even worse if its the "real game" and what you did before was just some kind of extended tutorial.
Bad crafting. When someone told me what you can "craft" in GW, I went all "... what the heck ?" I wasnt happy with the crafting options I had in other games, but what you get in GW is just plain silly stupid three times over.
Bad broker. I want a worldwide broker with a good search option. Nothing else will work. Give it to me. Thank you !
Pointless timekillers. Like extreme times needed for travel. I dont need an "i win" button, but I definitely dont see stuff like traveling as an interesting task, and enduring it as an archievement.
No seamless world. For a MMO, I really want a seamless world to help the illusion.
Too small world. I absolutely loved the huge gameworld of Vanguard. Was the main reason I started playing that game in the first place.
Game breaking bugs that dont get fixed for ages.
1. Gear treadmills, i.e. way too much focus and emphasis on stats and gear upgrades.
2. People who whine that a game is too easy when they use a ton of mods and only roll with the top guilds.
