Ninja Gaiden on the hardest difficulty is next to impossible...and that's from somebody who beat Metal Gear Solid 1 and 2 on Extreme difficulty.
Metal gear solid 1 ive played japanese version and i dont speak japanese but still manage to finish and unlock extras and i dont go to sites for clues or use cheats if you think that hehe.
Games played:AC1-Darktide'99-2000-AC2-Darktide/dawnsong2003-2005,Lineage2-2005-2006 and now Darkfall-2009..... In between WoW few months AoC few months and some f2p also all very short few weeks.
there are lots of game that i had a hard time finishing actually. too many i couldn't remember some of them. so i'll share the latest instead. i had a hard time beating the police chase in need for speed undercover, seriously because the cops suddenly spawn everywhere when you heat level is really high.
another instance is in the silent hill games. specially the bosses. most of the time, i always ran out of health recovery items and bullets.
Battletoads on the NES, without a shadow of a doubt. Can't remember ever being able to beat it without an emulator with a save state function.
yeah me too, most NES games i played are on my emulator. this game is also really hard to be specially the speedy part of the game where you ride that freaking levitating whatever thing. hahaha. most of the time i always get thrown off that vehicle.
One of the hardest was one of my first: Panama Joe (or Montezuma's Revenge on all systems other than the ZX Spectrum I played it on). When I first beat it I was unbelievably stoked.
More recently: FFX trying to unlock the dark Aeons. I couldn't get them all, and it still haunts me.
Special mention for Gothic (the first one) as the controls made my head explode. Just couldn't get used to them.
I've seen Battletoads pop up a few times and dear god am I forced to agree. I remember having so much fun until reaching the third stage - racing obstacle course from hell.
FInal Fantasy XI was for me the most difficult one I've played.
I've tried to start a couple of times, but simply could not get into it. No good quest info (you had to guess from some quests where you had to go or what you have to do - the quest-giver nor the quest description just gave too little information). And that boring grind. It'd take weeks before you get to a new level wher eyou could get some new skills. And when you start playing and have only like 2 or 3 skills you can use and are stuck with it for a month or 2 before you get ONE additional skill..?
have never played a "difficult" MMO out of the ones i've played.
For non-MMO i'd have to say old text adventures for the Spectrum 48k and C64
more recent?? ok then, the X series specifically X3 if eve was more like this i'd have been sold in a big way and lifetime subbed. however it's not as good (my opinion) so i don't bother with eve.
Thats probably the most difficult game I ever played, I werent even able to get past the first 10 minutes of gameplay - lol.
So I gave up there after and never touched it since. Probably just did something wrong, but yeh thats pretty much the most difficult game I ever played.

Dwarf Fortress is hard, gave up on it quite early unfortunately
The Witcher with the FCR mod had me screaming with frustration now and then..
Ultima Online during the first few months after it debuted. Nothing else comes close now or since.

What with the constant disconnects, huge lag and 56k dial-up speeds, I'd have to say UO was probably the most difficult MMO've I've ever played

For me the most difficult game ive ever played is Shadow of the beast. i almost thorw my controller that time everytime i played that game.
