How to un register???

in Final Fantasy XI

hi. i got my credit card bill today and it still says its charging me for Play online. However, i dont have my ID or password anymore, and i cant find a place where i can retrieve my information. i would call play online but it says i have to have my member id at hand.

 

anyone know where i can retrive the information of my ID and Password through putting my email address or something on their over complicated website??

 

any help you guys have would be greatly appreciated
