Counter Strike. No joke, it's the most skill-based online game you'll see. So skill-based you will actually rage quit after 2 hours with scores like 1-10, 2-15.

Agree completely with what fingerfight says, except that I'd encourage you to stick with it. Keep trying and you'll get better at CS so long as you aren't a fail gamer. But yeah, expect to get raped at lot for the first few weeks/months.
Guildwars, it is one of the few games where skills matter more than gear. It looks like GW2 will be even better, but since it isn't released yet we have to wait and see there.
Darkfall is a good skill based game once you get your chatacter stats up high enough... As you asking in an MMORPG forum I guess your not to oworried about lvling up and developnig your character as thats what MMORPG are about... So yes the darkfall combat at high lvl is basically all skill based, sure some use macros to euip sword and board really fast and few other things but nothing stoppnig u doing that as well..
Another good game for skill based PVP i guess is Age of Conan esp at the different lvling throughout the game.. plus of course its free to play now.. to pull of combos you need to hit a combinationof different keys and can be quite hard to pull off at the right tiems so yeah you need at least some lvl of skill to be able to pvp well in that game.
If you mean "real skill" to define only twitch/current combat and aiming with no outside variables/longer term macro tactical skills then there are no skill based pvp mmorpg games. Dedicated pvp games are really the only way to go.
If you mean skills to include tactical skills, planning and the like then any good open world game will do.

Exactly.
I, too, would highly recommend Guild Wars (1). It's the most balanced game pvp-wise on the market these days, as it seems. There is absolutely no gear grind and only your skill and knowledge of the game mechanics / classes matters.

If you want to try something that is not an fps or rpg, then you can try bloodline champions. The installation takes only about 200-300 megabytes, and it is free to play. Its a quick arena based pvp game where one match takes about 5 mins, and it has 22 different classes to choose from.

I fully agree with this. If it's an MMO type game with real skill required for PvP; then you are looking at Guild Wars. The best part is no sub, its buy to play. Everything else can be earned in game.
guildwars is a contained, restrict arena type pvp. You pretty much know whats coming, you know the landscape, you can prepare for it, set a strategy.
Real pvp is open world pvp where you can find yourself in a situation that isnt pre-determined, un-expected, where you might not have prepared for every contingency, a test of your ability to adapt and overcome a situation where nothing is set in stone, where the territory may or may not be familiar. You might be outnumberd, out classed, out gunned... but with tenacity, initiative, quick thinking and wit, ie, pure skill... you can still overcome the odds.
Limited, 4v4 or 8v8 pvp is more about predicting patterns in a cocoon envoiroment where every team eventually falls into some kind of cookie cutter build which inevitably degenerates into rock paper scissors.
Thats all guildwars became in the end, just 4/5 recognised builds that everyone used, the only time they changed was when the DEV's decided to alter skills and shape the face of pvp. There was player choice, but ultimatly, the way the dev's alterd skills pretty much forced a style of pvp that they wanted.

Are you talking about ganking ? I see your point though, but in a controlled environment it comes down to skill and strategy which I find most important, whereas in an open world pvp situation it is often the case that you get ambushed at a time where you have absolutely no chance of getting out alive. I mean, I love open world pvp when it is at least a bit fair, but most of the time its just people back stabbing you when they see you have no chance of defending yourself.
Real Skill PVP in an MMO does not exist that I can know of.
I started out with Tribes and Counterstrike among others and never have gotten into any MMO PVP because it just always seems to me like you have a built in Aim-Bot hack in the games.
Advantage in MMO PVP is always taken by being higher in numbers, or level, or gear, and it more about the Gank than the Strategies or Skill win.
Problem with FPS games is the abundance of hacks for any of them. That is the main reason I rather play a PVE MMO anymore. PVP does nothing for me until someone makes something different that actually works instead of a gankfest.
If you're looking for REAL PvP with skill, check ACE Online. Remember Star Fox 64? Well, imagine that, but with better graphics, MMO gameplay and dogfighting against other players in ships. Since you're playing against up to hundreds of other players in the same time, you need skill heavily if you want to compete.
don't expect to be able to do this in any short period of time, but here is what is possible in eve. http://www.youtube.com/watch?v=nSoFCC26KGw. No way in hell could I or anyone I know pull that off, regardless of ship/skillpoints.
EDIT: also I would turn down the music... no accounting for taste . also look up some videos for nano gangs in eve. Skirmish style battles at long range, null sec blobs, small gang pvp. All kinds of different pvp, all open world. It is a slower paced game, but I've never had my adrenaline pump in pvp like it does in eve, except maybe in shadowbane. Great fun but hard to get into without help from a corp willing to show newbies the ropes. PM me if you want a buddy invite.

Try Darkfall
I wouldn't suggest Darkfall or any other MMO at all actually. If you want player-skill-based PvP then you should look out for a FPS instead.
Sad but true.

By real skills you mean who can bind hot keys better and click mouse faster while running in circles?

Im currently using League of Legends for my PVP fix, theres really not much better around.
