You can die and have the money to replace all your items in EvE. Some people buy items they can't afford to replace tho...
On a related note, I was on a pvp roam and we caught a PvE battleship, we demanded that he sing for us on teamspeak to keep his ship. Initially he was resistent but then another guy randomly came in while we were still holding this guy, we violently killed said guy in front of him. He changed his mind and sang "I'm a little teapot" and we let him go, so yeah sometimes risk is avoidable, I wouldnt go as far as to say 100% of the time tho. This dude got to keep his ship in exchange for the loss of a bit of self-respect.

It still makes me shit bricks when a cyno beacon is lit in front of my internet spaceship and enemy internet spaceships start multiplying next to me. In fact this happened yesterday and I just barely made it out alive (100% shield... 80%... 50%.. 20%.. FUCK!, ALIGN! MICROWARP DRIVE ON! SPAM THAT WARP BUTAN!). It did get the blood flowing tho since it was somewhat unexpected, its that "then suddenly rape" factor that makes a dull situation very exciting.

I think people are looking at full loot the wrong way. Its not about the gear. Thats why losing stuff occasionaly isnt a big deal. You would think that would entice more people considering all of the "so tired of gear-treadmill theme park" whining that goes on. Full loot also creates the most realistic economies in any MMO.

Although I do agree gear based is good and for reasons that are fairly staightforward I dont agree with a few of your points.
1. Diablo is not a success, its not even out yet. The old version was a success because its was virtually the only game like it (and I am not refering to its gear focus).
2. progression and gear are not directly realted. They CAN be related but they are not interdependent as you are suggesting.
