It's bad design, pure and simple. Looking back at EverQuest I can think of Emperor Ssrasheva, which was an epic battle before other expansions came along and made it a cake walk. Originally you needed about 6 groups for the 6 adds that respawned instantly upon death, and then the Emperor himself who could wipe out tanks in one decent flurry, so you had to have multiples lined up to take over if the first one went down. Great encounter.
Then there was another one in Plane of Nightmares (think it was that zone) where during the fight with this Witch type character, she would spawn a load of creatures to come and help her, and they'd swarm all over the raid, multiple times depending on how much health she lost. Then the Worm in Luclin - can't remember exactly where - that had dozens of smaller worms crawling around, and more came during each step of the battle.
There's a lot of things developers can do if they put their minds to it. However, judging by the effort placed on modern MMO's it's no surprise all we do is stand there hitting a mob until hitpoints = 0. They can barely get the rest of the game worth playing, what makes you think raids are going to be any better?
