There are two routes you can go with that. You can pick Sword/Blood or Chaos/Fists. Sword and Chaos would be the tanking side, boost the skill levels in Survivability and add gear to match. Blood and Fists have the healing and mix well with the other two. Sword/Blood works well together as does Chaos/Fists.

After playing a Templar Preacher class, I'd have to pick the Sword/Blood, as the Sword line has an early ability that boosts your defense while a passive attached to it gives you regeneration. That mixed with Blood for afflictions and shields gave me a lot of survivability.

I'm not sure if a hybrid tank can really survive a dungeon, though, but for soloing it should be a nice mix. Also, both Chaos/Fists and Sword/Blood can pump out some nice DPS if you want a change at some point.

There are definately some tank/heal specs that are fairly solid, however they will take pretty deep specs into trees to unlock.

The question is, how far into your build are you?  How many AP are you looking at for this build?  Are you wanting a more solo type build, something to pvp with, or a end-game dungeon tank?

For soloing, I highly suggest the sword/fist.  It has some good mitigitation, dps, and pretty nice self healing.  It can be unlocked at the first teir with 60 AP (http://tswguides.blogspot.com/2012/06/my-first-60-inner-wheel-survival.html), and we also have a 400 point version (http://tswguides.blogspot.com/2012/07/400-point-clawblade-survival-and.html).  The 400 point version with a few tweeks is viable to solo with at Q10.

If you are wanting an end-game tanking build, well I know Ryahl was working on one, however we haven't gotten into doing that too far yet.  Our build page has a number to choose from, however I don't believe any are true "heal tanks".

Well blood magic has damage and heals, and is very...mage like in this aspect.

Theres the green miscellaneous skill sets offer a hate generating and provoke skillset, they do not require a weapon.  This means by using the hate generating passives and the range taunt, you could technically tank with any weapon set...perhaps not as a viable main tank for nightmare dungeons but would suffice as off tank or for normal dungeons.

I have seen fist/blood characters tank and heal themselves.

Fist offers some heals but is melee and not really mage like.

My suggestion would be to either get familair with the miscellaneous tank skills, see what works as far as pairing with what skills you want to use...remember that the gear you wear will allow you to have the stats to tank, so gear up with obvious tank gear, you probably wont do well in the damage department.

Or you could consider chaos magic as magic, despite it playing out like kung-fu mand to hand combat...which specializes in evade tanking.

You would have to prove yourself as a mage-tank-healer most likely, especially if you wanted to tank nightmare dungeons.  Im not saying its possible to get exactly what you want but im also saying its not impossible...i dont think people have gotten too deep into non-typical roles and related weapon sets...it seems people still cling to what appears to be the "best" tank combos ect.
