Try some sandbox MMO's. I guess there are only two that aren't horrific. I found a game that was very refreshing from the same old, Darkfall UW is that game. I has a real old school MMO feel to it. There is danger, freedom and consequences to your actions. There's some old school grinding and no hand holding, good times.
The world is typically big enough that you can run off the grid and not be bothered every 2 min by PvP gankers.
Just a thought, if you are sick of hand holding and being treated like a child that get's ice cream regardless if you make the right decision or not.
Edit: Also the world is mostly seamless and you can climb, sneak and explore to your heart's content.
Well there's Vanguard, it has a pretty open world but the sense of being "lost" is kind of dependent on you. There are quests and some have markers some don't, but the fast travel...*sigh* why they added it is beyond me. You can fast travel practically anywhere without having discovered it, and I've still yet to find a really big hub of players in the game they're all just spread everywhere.
HOWEVER!! With that being said I will say this is the only MMORPG as of late to give me the wide open feel an MMO should. There are practically no loading screens (Ya a 9 year old technology that STILL needs mentioning), and the dungeons aren't instanced. I float in and out of this game as it is a bit difficult to solo in, but if you want more of an open world feeling, with plenty to see and explore then this is it.
Also, single player wise you could easily try Witcher 2, the only reason I stopped playing was a fight with another Witcher early on that's a tad out of my skill range and was not prepared for in the slightest for. I know I could easily reload a previous save but I'm stubborn.
Vanguard: grab your favorite class and start running in any direction. you will die, you will explore and find open dungeons. or just start crafting and harvesting, or play the ambassador with the Diplomacy options.
Guild Wars: while highly instanced this game had a compelling world and lore for me.
Guild Wars 2
The Secret World: combat sucks. but the riddles and puzzles and storyline are quite nice.
RPG's:
have you tried just a sandbox builder something along the Minecraft route? there are a variety of these types of games out.
Baldur's Gate while quite old school is pretty famous for its depth and world. and they just released HD textures for the game as well.

Go ahead and try out Dark Souls
it's really one of the best RPG if the THE ultimate best., the port to PC is shitty tho, you'll need DSfix, DSMfix, DSCfix
i recommend the PC version if you have a good PC the graphics are just amazing. go for 360 if you don't.
