I use a password management application. There are several available. Most cost money but there are a few decent free ones available. Look on sourceforge.net for a some free open source options.

I use a print out I made with a back up on a U.S.B stick just in case I loose the printout so that I can make another. All random capital and lower case letters + numbers + special characters for everything. I don't re-use passwords. Combined with a clean email for publisher sites, game registration (never re-used for anything else). I don't do any real world stuff online. Banking, paying bills ect.. All pre-paid cards so there's not much of a risk for me as far as anything I've put out there.
I'm leery of keeping them on my HD. What I write them down in is in the back of an old copy of a Windows 95 manual, hehe. Back from the days when you got an actual book! But I do admit it sometimes takes me a while, flipping through page after page, to find a password that is old or obscure. My writing is sometimes not that great, either.
