You do realize that people have different levels of internal activity that they are aware of and unaware of? It's perfectly possible to visualize conversation choices as a wheel of options while talking to someone. It would be analogous to visualizing a place that you've visited before while talking about places you've visited. Instead of visualizing an image of the conversation topics, these people are visualizing their conversation options, in the format of whatever the Mass Effect wheel looks like.
There's also a wide range of how people visualize things. Some people think in words, others in pictures. Some people even hear in color. There really is no benchmark for this type of thing. It's probably why some are so affected by it while others can't believe it could ever happen.
The closest I ever came to this was during my late teens. I was borderline addicted to the old Sega Genesis Shadowrun game and played it virtually non-stop when I could. There was a very distinctive alarm that triggered inside of megacorp buildings and I heard it a lot. Even when away from the game it was as if the alarm was sounding just at the fringe of my hearing. Very annoying but I guess it's better than hearing "Passcode: Rejected" over and over again.
I've played many games where quest objectives glow/sparkle/etc. I see it everywhere and think "I need to get there/that!". I'll be walking somewhere and see those sparkles..Drives me nuts but it's nothing big.
